Heart failure is a complex clinical syndrome and a serious stage of a variety of cardiovascular diseases.It has a high incidence and a severe prognosis.The 5-year survival rate of CHF patients with clinical symptoms is similar with that of the malignant tumors.Heart failure is becoming one of the most important cardiovascular diseases in the 21st century.So the prevention and treatment of CHF is becoming more and more important.ACC/AHA,ESC published guidelines of CHF in 2005 respectively.ESC published a new edition of CHF guideline in 2008.Our country published a recommendation and a guideline of CHF in 2002 and 2007 respectively.These recommendation and guidelines play a important role in the prevention and treatment of the CHF due to cardiovascular diseases.The treatment strategy of CHF includes pharmacological therapy and devices therapy.The drugs includes neuro-endocrinic inhibitors ACEI,ARB andβ—blocker and Aldosterone antagonists,Diuretics and Digoxin.In this article,we report a case of heart failue that was treated in cardiac ward of our hospital.The patient met with an acute worsening of heart failure after one travel in which she got tired and caught a severe cold.At the beginning,the patient got a short treatment in the emergency ward.The emergency instructions includes stopping theβ-blocker,increasing the dose of diuretics,using the nitrate esters and inotropic drugs by intravenous route and antibiotics for the anti-infection. When the patient got more stable,she was sent to the general ward of Cardiology.Then the physicians gave the instructions that using antibiotics to control the infection so that we can eliminate the cause reason of the worsening of the heart failure,using an ARB to improve the prognosis of the CHF and to control the level of blood pressure,using Aldosterone antagonists which was added to existing therapy to improve the prognosis,using the Diuretics and Digoxin to improve the clinical syndromes and using drugs to control blood sugar changes,as well as the general nursing such as administering oxygen,electrocardiogram and blood pressure monitoring,restricting the intake of the water,sodium, lipids and sugar.Two weeks later,the patient got well and left the hospital. In this article we would discuss the patient's clinical syndromes,the changes of condition,the treatment of the drugs in detail,and discuss the drug treatment of the CHF,the evidences of the drugs used and other questions-related.
