| Objective: The functional obstacle to ovulation is one of the important factors causing women to be infertile. According to her clinical experiences for many years, on the basis of traditional Chinese medical theory, Professor Li Shuling developed kidney-benefiting and conception-aiding granule which can invigorate the fuction of kidney and the spleen and negulate the "Chong" and "Ren" pulse, recuperate qi and invigorate blood circulation. This study is aimed at observing the clinical effect and mechanism of this kind of medicine when treating the functional obstacle to ovulation diagnosed deficiency of kidney yang,and providing scientific basic for the clinical application of it.Methods:60 patients were divided into two groups randomly as treated group 30 cases and controlled group 30 cases.The treated group used kidney-benefiting and conception-aiding granule,and the controlled group used western medicine clomiphene citrate. Clinical observations were carried out on the two groups before and after using medicines respectively, complemented with routine gynecological inspection. They were studied by radioimmunassays for female hormone such as follicle stimulating hormone (FSH), Luteinizing Hormone (LH), Estradiol (E2), prolactin (PRL),testosterone(T) and Progesteron (P) and by B type ultrasound merchine monitoring the development of follicles and endometrium before and after treatment.The animal experiment applied hydrocortisone way to replicate deficient kidney yang female rat model,devided them randomly into six groups:normal group,model group,high,middle,small dosage of kidney-benefiting and conception-aiding granule group,control group. 10 rats each group.Observing and comparing the symptom of deficient kidney yang ,the ovarian morphology ,and the endocrine determination during all the groups. Results:The clinical effect: The pregnancy rate of the control group was 56.67% while that of the treatment group was33.33%(P <0.05). The following symptoms such as irregular menstruation, lumbago and dizziness have been effectively regulated among the treatment group obviously better than the other group treated with western medicine. Dynamic ultrasonography observations in succession showed that the ova and the uterine membrane obviously increased after every time phase (P<0.05～0.01) and they were higher among the treatment group than the control group. The ovulating rate of the control group was 73.33% while that of the treatment group was 66.67%, FSH,LH,E2,P level increased(P<0.05～0.01) and PRL level decreased significantly(P <0.01) in both groups, there were no significant differences between the groups(P>0.05). The total effect rate of the treatment group was 76.67% while the rate of the control group was 80.00%(P >0.05).The results of the animal experiment manifest that kidney-benefiting and conception-aiding granule could improve the symptom of deficient kidney yang significantly, increase the weight of uterus,ovary,adrenal gland, improve ovarian morphology, and promote follicular development, comparing with the model group.There were different effects during three dosage treatment groups.LH,E2 level increased in the treatment group better than that in the model group(P <0.05).Conclusion: The kidney-benefiting and conception-aiding granule has sound curative effect on the functional obstacle to ovulation and is superior to the common western medicine clomiphene citrate in the respect of improving the symptoms of deficient kidney yang, regulating menstrual cycle, which reflects the traditional Chinese medical thought that "menstruation should be regulated before pregnancy." The granule also has the good function of regulating the Hypothalamus-Pituitary-Ovary-Uterine Axis and promoted follicular development and endomembrane thickening, increasing the weight of uterus,ovary,adrenal gland, and improving ovarian morphology,so as to promote ovulation and pregnancy.
