Objective:Perinatal health care especially intrapartum health care is directly related to the health of mother and infant. Study shows the delivery outcome be different according to different infanticipate position. Proper position has certain significance in promoting labor, improving vaginal delivery rate and decreasing maternal and neonatal complications. Now there are many positions in clinic , such as supine position, sitting position and free position. No microseismic sitting position is reported. The aims of this study were to explore an ideal way of infanticipate position through observation of the time of labor , cesarean section rate , the Vas score , amount of postpartum bleeding and neonatal outcomes of different positions.Methods:From March 2008 to March 2009 in Jinan central hospital, 180 pregnant women without any vaginal delivery taboo were divided into three groups randomly, 50 cases in microseismic sitting position group(A),50 cases in sitting position group(B),80 cases in supine position group(C). Inclusion criteria:(l)Aged:22 ~ 30years old; (2)Primipara; (3)Correct gestational age; (4)Single pregnancy; (5)37~ 41gestational weeks; (6)NO premature rupture of membrane (PROM); (7) Engagement of fetal head; (8)NO cord around neck; (9)NO placenta previa, no placental maturation, no amniotic fluid volume abnormality;(10)Correct external pelvic measurement;(11)Law of uterine contraction, smooth making progress of labor;(12)No pregnancy complications, no relessive fallen umbilical. There was no significant different of maternal age, gestational age and weight among the 3 groups. The time of labor , cesarean section rate , the Vas score , amount of postpartum bleeding and neonatal outcomes were compared among the three groups.Results:(1)The time of the first stage and the active phase were the shortest in microseismic sitting position group(P<0.05 ) . (2)The cesarean section rate was the lowest in microseismic sitting position group, and there was significant difference between microseismic sitting position group and supine position (P<0.05 ) . The amount of postpartum bleeding was the least in microseismic sitting position group(P<0.05 ) . (3)There was significant difference of Vas between with or without the infanticipate chair when the dilatation of cervix in 6~8cm . (4)There was no significant difference of neonatal conditions among the 3 groups (P > 0.05 ) , neonatal asphyxia was mild.Conclusions:(1)Compared with sitting position group and supine group, the time of the first stage and the active phase were the shortest in microseismic sitting position group, which shows microseismic sitting position can accelerate labor progress. (2) Microseismic sitting position can decrease cesarean section rate. (3)The Vas displays microseismic sitting position can reduce labor pains.
