| Objective: In this article we investigated the effect of therapy for Hypertrophic Scars withβ-ray and the Changes of Serum TNF-a Levels.And we concluded that TNF-a was involved in the pathogenesis of hypertrophic scars, the determination of which was of diagnostic and prognostic value in clinical practice.Methods: 1. In ray group,42 patients With Hypertrophic Scars are treated withβ-ray in the same doses(5.97Gy/cm2 )and methods.Ray group's average age is 29.4±8.6,In control group, 32 patients With Hypertrophic Scars are divieded into scar group and laser group. In scar group, 12 patients With Hypertrophic Scars are not treated with any methods, scar group's average age is 26.8±10.4,In laser group, 20 patients With Hypertrophic Scars are treated with CO2 laser in the same doses, laser group's average age is 30.2±9. 8. In health group ,35 normal subjects, their average age is 29.5±6. 5.2. All patients are examined about the changes of scars before therapy applied and after applied therapy.Comparing all the Results and judgments based on statistical analysis, we drew some conclusions.3. Serum TNF-a levels were measured with RIA in all patients with hypertrophic scars before treating and after treating, and were analyzed with t test. Results: 1. The serum TNF- a levels in hypertrophic scars patients were (13. 91±4. 57) fmol/ml significantly higher than those in control group (6.74±0. 81) fmol/ml (P < 0.01).2. The result showed that 35 cases were cured in ray group,the cure rate was 83. 3%; 7 cases were effective, the effective rate was 16.7%; All cases had no wound infection and radioactive complication.In control group , 12 patients of scar group had inefficacious results; Among the 20 patients of laser group, 6 patients'scars were healed (30.0 % ) , 5 patients'scars improved (25.0%), 9 patients'scars had no changes (45.0 %) . In ray group, the cure rate was 83. 3%, the all effective rate was 100%; In control group , the all cure rate was 30.0%and the all effective rate was 55.0%; the ray group excels the control group, differences being statistically significant (x2=24.09,21.15, P<0.01).2. A statistical comparison between the results before therapy and the first applied therapy was not significant in Serum TNF-a levels (P > 0. 05) . But between the results before therapy and the second applied therapy, there was statistically difference in Serum TNF-a levels (t= 15.63, P < 0. 05). After the third applied therapy, Serum TNF-a levels were significant lower than those before applied therapy, there was statistically significant difference in Serum TNF-a levels (t=22.51, P < 0. 01). After the second applied therapy, difference being statistically significant between the ray group and the control group(t=26.13, P<0.01). But in the control group, the scar group excels the laser group, a statistical comparison was not significant in Serum TNF-a levels(P >0. 05).Conclusion:1. TNF-a is an important cytokines in the formation of Hypertrophic Scars.The serum TNF-a might be implicated in the development of hypertrophic scars, and could be used to assess the effect.There is a negative correlation between TNF-a and the effect, the determination of which was of diagnostic and prognostic value in clinical practice.2.Theβ-ray contact therapy is a simple, effective treatment of Hypertrophic Scars with low expenses and little time.It has good clinical value and it is worth making it a common practice in the clinic. |
