The Clinical Analysis Of 52 Cases Of Crohn Disease (CD)

Objective:To get message of current situation of Crohn disease'diognosis and therapy of our hosipital,and to analyze its clinical character,the endoscopy and pathologic histology character,to prvide experience for diognizing and curing Crohn disease.Methods:Adopting retrospective analysis mode analyze 52 crohn disease cases of diagnizing in the second xiangya hosipital of CSU during 2005.1-2008.12,alalyzing the clinical character,laboratory examination,imaging findings,manifestation of endoscopy examination and pathologic histology,and mading contract with clinical data of home and abroad.Results:Bellyacke,diarrhoea,abdominal distention are the commonest clinical character of Crohn disease.Cases like fever and weight loss occur high than 20%,night sweat fewer than 10%.7.69%cases have tuberculosis infection of antecedent history or in existence.Gastroscope examination 13 cases,positive ratio is 2/13,gastroscope biopsy 1 cases,positive ratio is 0.Through barium meal exzamination diagnizing CD is 7(53.85%).The magic manifestation of enterosarcoma biopsy are: skip lesions(14.63%),deep ulcers(14.63%),fissures(17.07%),cobble-stone appearance(12.20%).The positive ratio of enterosarcoma biopsy is 3.33%, operation collection is 56.00%.The affection draging in only small intestine is 18(34.62%),draging in small intestine and colon is 24(46.15%),draging in only colon is 9(17.13%).The continue theropy ratio of patients after operation are 45.83%.Conclusion:The clinical character of crohn disease has no specificity,the magic are bellyacke,diarrhoea,abdominal distention; draging in small intestine and colon is the commonest stype,the most affection of colon is ileocecal valve;barium meal,computerized tomography,capsule endscopy,intestine endscopy have important contribution for diagnizing intestine affection.Enteroscopy exzamination is the most important for diagnosis of colon CD and terminal ileum CD,but for untypical affection it is hard to diagnize.The positive ratio of enterosarcoma biopsy and operation collection both are low,but the first is high than the after.For the cases lacking pathalogic evidence and diagnosis of CD,removing other definite cause colonitis is important.
