Objective To compare the therapeutic effectiveness of transplanting bone marrow-derived mesenchymal stem cells(MSCs) in the heart of myocardial infarction(MI) via extraepicardial,intracoronary and intravenous injection, and find the best way of MSCs transplantation for MI.Methods In a MI model of swine, the second diagonal branch of LAD coronary artery was occluded by gelatin sponge. Swines with MI were randomly divided into 6 groups, 5 swines in each group. MSCs were labeled with BrdU and transplanted after 4 weeks. Extraepicardial injection group: 5ml(1×10~7)MSCs suspension was injected directly into the MI region of each swine; Intracoronary injection group: 5ml(1×107) MSCs suspension was injected into coronary artery of each swine; Intravenous injection group: 5ml(1×107) MSCs suspension was injected into auricular vein of each swine; 3 control groups: 5ml DMEM was injected into eacth swine by 1 of the 3 ways. The therapeutic effectiveness was evaluated with cardiac color ultrasound and cardiac magnetic resonance imaging. The histomorphology changes of MI areas and distribution of transplanted cells were analyzed by histopathological examination.Results Compared with each control group, the left ventricular wall was thicker,infarct area was smaller and cardiac function was significantly improved in all MSCs-treated animal(sP<0.05). Furthermore, compared with intravenous injection group, cardiac function was more significantly improved in both extraepicardial injection and intracoronary injection group(P<0.05). However, there was on significant statistical difference between these two groups(P>0.05). The scar tissue was less in all MSCs-treated animals than each control group. The MSCs labled with BrdU were viable in every transplantation group. Compared with intravenous injection group, the MSCs labled with BrdU were more in both extraepicardial injection and intracoronary injection group.Conclusions Cardiac function of MI could be improved by different ways of MSCs transplantation. But extraepicardial injection and intracoronary injection were more effective than intravenous injection. Intracoronary injection was the best way of MSCs transplantation for MI.
