| Objective:To evaluate the safety and efficacy of vitrectomy combined with phacoemulsificafion and intraocular lens imlantation for the treatment of rhegmatogenous retinal detachment.Methods:We retrospectively reviewed the the relativly complete medical results of 86eyes (86 cases) with complicated detachment of retina combined with different degree lens opacification and Vitrectomy combined with phacoemulsificafion and intraocular lens implanta- tion from March 2005to December 2008.Preoperative and postopera- tive acuity and com- plications are recorded.Results:1 acuity of vision: Pre-BCVA 0.1-0.3 was 16 eyes(18.7%) , Post-BCVA 0.1-0.3 was 34 eyes(39.5%). Pre-BCVA>0.3 was14 eyes(16.3%), Post-BCVA>0.3 was 20 eyes(23.3 %); The Post-BCVA was better than the Pre-BCVA 72 eyes(83.7%),was unchanged in 10 eyes(11.6%),and worse in 4 eyes(4.7%)。2 The retinal reattached rate: The retinal reattached rate 83.7%(72 eyes) through only one time operation, 9.3% (8 eyes) through the seconed operation, 93.0%(80 eyes) through the last operation.All of the patients retinal were anatomy reattached in operation. among the total, revelation omission slit pores in preoperation 56 pieces(24.1%). Around mostly slit pores, rather obviously desmo-generation traction was seen under the operating microscope. Anterior chamber effusion 12 eyes(13.9%), we can see anterior chamber brilliance, there is the cellulose effusion epi-lens in the pupil area. To mydriasis, after pantis and all over the body ulilize steroid hormone therapy, the inflammalion was control, the effusion was complete absorption at last. Anterior chamber hematocele 14 eyes(16.3%), to consider the errhysis at the comea opaca nick, the effusion gradually absorption after to take Yun nan white powder. Multiplicity hematocele of uneasy absorption to use anterior chamber bathe, in ofder to avoid to form blood-staining of cornea. Tempore intraocular hypertension 12 eyes( 14.0%),Generally, to quiescent point quickly manicol, to take Acetazolamide, to use dump eyedrop etc. in part, intraocular tension can recover normal. Follow-up period, the intraocular tension was normal limits all the time. Posterior synechia 6 eyes(7.0%),was limitations, to have anterior chamber effusion to do with, to take mydriasis, local application hormone and antiinflammatory eye drop, the pupil was to recover normal, light reaction sensitiveness. After-cataract 15 eyes(17.4%), severty eyes to use Nd:YAG laser meta-capsotomy. New slit pores form 6 eyes(7.0%), all of them on the ambitus, to laser photo-coagulation to block tears retinal, to reinforce retina, to avoid multiple operation.PVR 4 eyes(4.7%), vitrectomy one moretime, the silicone oil was filled in the eyes.Conclutions: vitrectomy combined with phacoemulsificafion and intraocular lens imlantation for the treatment of rhegmatogenous retinal detachment is a way of the safety , efficacy and feasible. It can recover quickly visual function of patients, shorten recovery time of patients postoperation, avoid operation one more time, lessen the burden of patients'mental and economic pressure.
