BackgroundIn 2003 HIV prevalence was expanding fast. By the end of June 2003, the cumulative total of reported HIV positives was 45,092, including 3,532 AIDS cases and 1,800 recorded deaths. Especially some key areas have appeared summit of morbidity and mortality. In order to response the challenges of AIDS, the Ministry of Health (M0H) published the guidelines on the China comprehensive AIDS response (China CARES). Firstly 51 China CARES were established throughout the country, then 76 China CARES were set up in 2004.Advocacy for policy support, general population HIV awareness campaigns, surveillance, testing, voluntary, behavior change interventions, prevention of mother-to-child transmission, treatment, care and support were carried out in China CARES. In 2008 the M0H carried out a final evaluation to assess the implementation of the HIV policy and HIV control and prevention work, to scientifically evaluate the effect and influence of the HIV control and prevention work, to draw lessons and successful experiences from China CARES, to provide successful experiences for nationwide HIV control and prevention. Evaluation content includes the implementation and effects of HIV control and prevention, analysis of cost of HIV control and prevention work and evaluation of successful patterns. In order to access the effect of treatment, care and support better, designing and carrying out this special survey on quality of life of AIDS patients.ObjectiveThe overall objective of this survey is to investigate the quality of life of adult AIDS patients in different areas including China CARES and not, to study the effect of treatment, care and support on the quality of life of AIDS patients. The specific objectives of this survey is awareness of the quality of life of AIDS patients in different areas, survey of relative HIV service providing and being made use of in different areas and analysis of related factors on quality of life of AIDS patients. Object of study and methodThis survey was carried out in the China CARES which has the most AIDS patients having accepted the Antiretroviral treatment(ART) in Henan, Yunnan, Guangxi. Simultaneously, control group was chosen from the China CARES around, which has similar community economic with the China CARES, the most AIDS patients and non-Global Fund county. The objects of this survey are AIDS patients the year between 18-55, who accord with the national standard for free antiretroviral treatment (ART) and being treated. The face to face interview was adopted. The questionnaires include the simplified Chinese version of Medical Outcomes Study HIV Health Survey, HIV awareness rate questionnaire from the Monitoring and Evaluation Framework issued by the State Council AIDS Working Committee Office (SCAWCO), personal basic information questionnaire and AIDS service providing and popularity questionnaire. All data were entered in the database set up by Microsoft Office Access 2003 and analyzed by SAS9. 0. Results572 AIDS patients were interviewed and 512 questionnaires including 268 China CARES and 244 non-China CARES were effective. The most of the patients are farmers, the age between 25-35, Han people and under Elementary School education level. Totally as to physical functioning dimension scores, the China CARES 68. 57 is more than non-China CARES 60. 25 statistically. As to energy dimension scores, the China CARES 53.65 is more than non-China CARES 49. 17 statistically. Other dimensions scores, a physical health summary score and a mental health summary score has no obvious difference. The quality of life scores have no difference between China CARES and non-China CARES which transmission mode is injecting drug use. There is significant difference in the dimensions scores including physical functioning, pain and social functioning between China CARES and non-China CARES which transmission mode is blood and plasma collection. Other dimensions scores, a physical health summary score and a mental health summary score has no obvious difference. The objective of this survey is to study the relationship between AIDS service providing, popularity and the quality of life of the AIDS patients. Specifically, the relationship between quality of life scores and AIDS service providing, popularity reflects the effects AIDS service providing, popularity on quality of life of AIDS patients. The pattern of follow up, whether knowing the local care institution, opportunistic infections treatment at one' s own expense and the frequency of using condom have something to do with the quality of life of AIDS patients.ConclusionNo matter the expense of time, devotion, working strength and covering surface, the China CARES surpass non-China CARES and provide valuable experience about HIV control and prevention for non-China CARES. The results of this survey show that in the dimensions social functioning and energy the scores of the China CARES is significantly more than non-China CARES. The dimension social functioning exactly reflects the care and removing discrimination against AIDS patients work and also reflects the effects of HIV control and prevention.
