Objective:Observing the influence of the Puerarin and Deer Melon Polypeptide Interventional Treatment(PDMPIT) to early rabbit Steroid induced Avascular Necrosis of Femoral Head (SANFH) serum nitric oxide(NO)content and Vascular Endothelial Growth Factor (VEGF).discussing the mechanism and effective of PDMPIT to early rabbit SANFH,to supply the scientific evicence for the clinical application。Methods:56 Japan rabbits(equal amount ingender),divid into 3 group,10 in blank group,23 in control group(modeling without therapy),23 in experimental group(modeling with PDMPIT ), Experimental group and control group accepted prednisone acetate gluteus injection,7.5mg/kg,twice a week, keeping 8 weeks, blank group didn't receive prednisone acetate injiection.All rabbit received penicillinum(80,000u per time) and Gentamicin(40,000unit pertime) gluteus injection,both were 2 weeks keeping. executed 2 rabbits randomly in every group 8 weeks later, pathological and historial observation of the dissected tissue, to prove the effect of the model. To obtain the blood from the ear edge vein of the rabbit, determining the content of NO by using the endpoint method. Feeding the 3 groups commonly,PDMPIT to experimental group after the time point and 2 weeks later respectively .6 weeks after the treatment,executed all rabbits ,obtained the vein blood and determined the NO content in blood serum, applying the pathological morphology observation of the dissected tissue, determining the positive VEGF osteoblast rate,positive chondrocyte rate, positive blood vessel rate.All date have been received analysis by applying SPSS13.0 statistic software, indicating the experimental data in mean and standard deviation( (x|-)±s)Results:in control group:One rabbit died of ulcer of alimentary tract and one died of otitis media in the second week modeling,One rabbit died of ulcer of alimentary tract in the 3rd week. In experimental group:Two rabbit died of ulcer of alimentary tract,One died of otitis media. Applying Cimetidine gluteus injection (0.84mg/kg) to protect mucosa of alimentary tract as a result.The model creating were successful,by proving the histological difference between blank,control and experimental groups,the average post model creation weight was lower than pre model creating, average weight in experimental group was increased after the therapy.Determination of NO content in blood serum expressed that:NO statistical difference between control and experimental groups before the therapy, both of whom had statistical difference between the control group(P<0.05) ,experimental group NO content was higher than control group(p<0.05) and, has not statistical difference beween experimental group and blank group(P>0.05), the NO content in blood serum had no difference between pre-therapy and post-therapy in control group.the same as blank group ,in experimental group NO content in blood serum of post-therapy were positively higher than prethepray. The status of blank group were normal. trabeculae of bone in femur head were thick ,dense, distinct in strcture, to line up in order in the shape of regular arch bridge, many osteoblasts in the age of trabeculae of bone, Large amount of Mature osteocytes in the bone lacune, bone lacuna vacuities could be seen sometimes, few osteoclasts and Large amount of normal proportion hematopoietic cell and lipocyte could be seen in cavum medullare. sparseness of trabeculae of bone, increasing of empty bone lacuna and enlargement and increasing of the lipocytes in cavum medullare could be seen in control group, the highest empty bone lacuna proportion could be seen in control group,proportion of empty bone lacuna in experimental group was close to blank group but higher than blank group, it is much lower than control group. The VEGF positive osteoblast rate , positive chondrocyte rate and positive blood vessel rate was highest in blank group, and lowest in control group. the experimental group was mediately, and there is statistical difference between control group and experimental group(p<0.05).Conclusion: causing the Steroid induced avascular necrosis of femur head and microcirculation disturbance of bone by using large dose Steroid.1 inhibited tendency of the persisting weaken expression of NO VEGF caused by the overuse of Steroid ,advancing the secretion of the cytokine such as VEGF ,thereby ,speeding up the revascularization and reossification of the femur head, improving the microcirculation of the femur head effectively, turning round the ischemic status of the femur head,promting the regeneration and restoration of the trabeculae of bone, promoting the bone strength, preventing the dysmorphosis of femur head.to prevent the occurring and development of the SANFH.
