The Research Of The Relation Between Renal Injury Associated With HIV And The Chinese Medicine Syndromes' Distribution

Objective:To research the situation in renal injury of human immunodeficiency virus-infected children by clinical epidemiology way.The relationship between the renal injury with age,HIV infection transmission,HIV infection stage,CD4 T-cell,the main syndrome were explored.Method:A cross-sectional study of human immunodeficiency virus-infected children in Areas of high incidence of AIDS of henan Province is adopted,which not received HARRT and not infectde by HBV,HCV. It is applied the clinical questionary of children syndromes in TCM framed by ourselves to collect information about disease history and TCM four methods of examination, its syndrome by TCM clinical expert,examine the main relation index.Statistical data processing by using SPSS13.0 software.Results:①To determine whether human immunodeficiency virus (HIV) infection is associated with incipient tubular or glomerular defects,we determined the urinary excretion of three low molecular weight proteins (LMWP):beta2-microglobulin (U-beta2-m),retinol-binding protein (U-RBP) and microalbuminuria (mAlb).The study involved 87 HIV-infected children, 62(81.6%)had elevated urinary excretion of at least one protein (Alb, U-beta2-m or U-RBP),At least two LMWP were abnormal in 57.5%(U-beta2-m and U-RBP),25.3%(Alb and U-beta2-m),19.5%(mAlb and U-RBP),An increased urinary excretion of all proteins was observed in the last18.4%.②We devide the patient into different groups according to age, HIV infection transmission,HIV infection stage or CD4 T-cell,the differences at different level have no statistical significance by One-Way ANOVA.③The relationship between age and mAlb,U-beta2-m,U-RBP have no statistical significance by Pearson Correlation Analysis. The relationship between CD4 T-cell and U-beta2-m have statistical signify- cance.The relationship between CD4 T-cell and mAlb,U-RBP have no statistical significance.④The results indicated that the incidence of AIDS syndromes in 87 children are ranked in turns as following:deficiency of spleen(50.6%),obstruction by dampness(44.8%),retention of phlegm(39.1%),deficiency of lung(36.8%),attackd by wind(34.5%),deficiency of kidney(19.5%),deficiency of both Qi and Yin(9.2%), deficiency of both Qi and blood(3.5 % ),diet indigestion(2.3 % ).⑤By Pearson Correlation Analysis,The relationship between U-beta2-m with deficiency of both Qi and Yin have statistical significance.The relationship between mAlb, U-RBP with deficiency of both Qi and Yin have no statistical significance.The relationship between mAlb,U-RBP,U-beta2-m with deficiency of spleen, obstruction by dampness,retention of phlegm, deficiency of lung,attackd by wind,deficiency of kidney,deficiency of both Qi and blood,diet indigestion have no statistical significance.Conclusion:There are mild renal impairment in human immunodeficiency virus-infected children.HIV mainly invades tubular.There is no relationship the renal injury with age,HIV infection transmission,HIV infection stage.The relationship between CD 4 T-cell and tubular have statistical significance.The relationship between tubular with deficiency of both Qi and Yin have statistical significance.
