ObjectiveTo investigate the relationship between the distribution of tumor infiltrating lymphocyte and the clinical outcome of the patients within colorectal caner liver metastase;to investigate anynew predict factors for patients with colorectal cancer liver metastase.MethodsSelected 75 cases of liver metastasis of colorectal cancer patients in Department of Surgery fo Fudan University Cancer Hospital from January 2006 to December 2007 all underwent primary surgical resection to obtain pathology confirmed,by CT or MRI or exploratory surgery to confirm the liver metastasis.Measurement and observation of indicators:for selected cases of patients with complete follow-up collection and follow-up deadline in April 2009. Application of mature technologies immunohistochemistry was determined in patients with primary tumor specimens and some specimens of tumor metastasis around the central parts of the tumor and tumor-infiltrating lymphocytes(tumor-infiltrating lymphocyte,TIL) CD3+,CD8+expression compared with TIL CD3+,CD8+ different parts of the expression and to study the distribution of tumor-infiltrating lymphocytes,as well as TILCD3+,CD8+in patients with positive expression and prognosis.Results:1.75 cases of liver metastasis of colorectal cancer patients five cases of lost,lost 6.7%,the shortest follow-up 3 months,the longest 36 months,with a median survival time was 16 months,1-year survival rate was 57.8%,2-year survival rate of 42.7%.2.Patients with colorectal liver metastases,tumor peripheral positive rate of tumor-infiltrating lymphocytes CD3+ 53.3%,positive rate of CD8+ 25.3%;positive rate of tumor central tumor-infiltrating lymphocytes CD3+ 48%,positive rate of CD8+21.3%.3.Tumor peripheral tumor-infiltrating lymphocytes in patients with CD3+ different expression show significant difference in prognosis(p<0.05),the other areas of tumor-infiltrating lymphocytes CD3 +,CD8 + expression has no significant impact on prognosis;according to whether liver metastasis resection,tumor-infiltrating lymphocytes CD3+ with different expression in patients show differences in survival (p<0.05).Conclusion: Liver metastasis in colorectal cancer patients,the radical surgical resection of liver metastasis is still the most effective way to improve the survival of these patients.So patients with liver metastases resectable should be given a surgical resection if we can. At the same time,for patients with inoperable metastases,and tumor-infiltrating lymphocytes in patients showed better prognosis,the prompt for such patients,we may do the treatment of immune-related efforts to improve the survival of patients.
