Objective:To study whether there is the correlation among microsatellite instability(MSI)in gastric cancer and tumor infiltrating lymphocytes(TIL)and neutrophil lymphocyte ratio(NLR).To reveal whether NLR levels and TIL degree of infiltration can predict the occurrence of MSI-type gastric cancer and thus guide the treatment of gastric cancer.Method:Study on 76 patients who have a clear pathological diagnosis of gastric cancer selected from December 2016 to August 2017 at the China Japan Union Hospital of Jilin University,gastrointestinal colorectal surgery department.There were 56 males and 20 females,35 patients over 60 years old and 41 patients less than 60 years old.All patients underwent routine blood tests and NLR values were calculated and divided into the NLR increased group(NLR?3)and the NLR decreased group(NLR<3).The tumor tissues of patients were collected and the infiltration of TIL was observed under light microscope with HE staining.According to the average count,the TIL infiltration degree was divided into 0~3 grade.The expression of MLH1 protein and MSH2 protein in tumor tissue was detected by immunocytochemistry.If the MLH1 protein or MSH2 protein was missing,it was defined as MSI.Both of them were positive defining as MSS.Using SPSS 20 statistical software for data processing.Analyze whether there is the relationship among gender and age and MSI detection rate,and analyze the relationship among NLR,TIL infiltration degree and MSI.Result:1.The proportion of MSI in female patients was obviously higher than that in male patients.The proportion of MSI detected in gastric cancer patients older than 60 years old was significantly higher than that in patients younger than 60 years old,of which the difference was statistically significant.2.A high degree of TIL infiltration is more likely to occur in MSI-type gastric cancer patients.And when the degree of TIL infiltration increased step by step,the detection rate of MSI also gradually increased,with statistical significance.3.It was statistically significant in the difference of which the proportion of TIL infiltration degree in NLR decreased group was significantly higher than that in NLR increased group.4.The proportion of MSI in NLR decreased group was slightly higher than that in NLR increased group,however it was not statistically significant.Conclusion:1.The degree of infiltration of TIL can be used as an index to predict the occurrence of MSI-type gastric cancer.The higher the degree of TIL infiltration,the greater the detection rate of MSI-type gastric cancer.2.The level of NLR correlated with the degree of TIL infiltration,but no correlation with the detection rate of MSI-type gastric cancer.
