ObjectiveTo explore the Traditional Chinese Medicine differentiation of symptoms and signs for classification of syndrome,to research the relationship of laboratory tests and clinical prognosis with the differentiation by the history,clinical symptoms and examination of severe hepatitis.MethodsRetrospective investigation and analysis of Affiliated Hospital of Hubei College of Traditional Chinese Medicine Liver Disease Branch (National Medical Center of TCM liver) from August 2006 to August 2008 of 127 cases of hospitalized patients with severe hepatitis and 220 cases of hepatitis in patients with ordinary(control group),sum up the Chinese medicine evidence-based distribution,according to TCM syndrome classification into statistical analysis.Main objection:①the frequency of card type of the severe hepatitis and the component ratio of the number of cases;②the distribution of severe hepatitis in the acute,subacute and chronic severe hepatitis③the relationship of prothrombin activity(PTA),serum total bilirubin level(TBIL),albumin(ALB)with each card type of the severe hepatitis;④the relationship of severe hepatitis TCM syndrome and clinical prognosis;⑤the comparation of TCM type distribution between severe hepatitis and general hepatitis;⑥symptoms and frequency distribution of severe hepatitis in different syndrome types of TCM.Using SPSS13.0 computer statistical software to analysis:count data using X~2 test (level of information on the use of rank sum test),measurement data using mean±standard deviation for descriptive statistics.P<0.05 suggests that the tests were significantly different.Results1.Fulminant hepatitis card is divided into four types:Liver depression and Qi stagnation type 30 cases,accounting for 23.62% of the total number;liver and gallbladder damp-heat in 35 cases,accounting for 27.56%of the total number;thermal subsidence 30 cases of cardiac type,the total number of 23.62%;of yin and yang from 32 cases of type summary,accounting for 25.20%of the total number.2.In one cases of severe hepatitis,only thermal subsidence type 1 cases of pericardial;subacute severe hepatitis in two cases,the liver and gallbladder from the heat and yin and yang of the summary type 1 cases;in 124 cases of chronic severe hepatitis,the stagnation of liver qi 30 cases of stagnation,34 cases of liver and gallbladder damp-heat,thermal subsidence 29 cases of cardiac type,yin and yang from 31 cases of type summary.3.In the PTA,the stagnation of liver Qi stagnation and damp-heat in liver and gallbladder showed no significant difference (P>0.05),yin and yang and heat away from the settlement must pericardial type card type and the other two more significant difference(P<0.05);in TBIL,the four card-based comparison between the difference was significant(P<0.05);in the ALB,the stagnation of liver Qi stagnation and damp-heat in liver and gallbladder showed no significant difference(P>0.05),thermal subsidence from the pericardium and yin and yang type summary comparison no significant difference(P>0.05),but thermal subsidence from the pericardium and of Yin and Yang must type the first two card type and more significant difference(P<0.05)4.Stagnation of liver qi and liver and gallbladder damp-heat in the improvement rate was significantly higher than the other two card type,the more significant difference(P<0.05),stagnation of liver qi and liver and gallbladder damp-heat in the non-efficiency(increase and death)showed no significant difference(P>0.05);thermal subsidence of the cases of cardiac-type increase was significantly higher than other three card type,the more significant difference(P<0.05);and yin-yang type of death from the summary of cases was significantly higher than other three card type,the more significant difference(P<0.05).5.Hepatitis cases in the general stagnation of liver qi in severe hepatitis was significantly higher than that,more significant difference(P<0.05);cases of severe hepatitis and liver and gallbladder damp-heat and fever subsidence pericardium and yin and yang-type was significantly higher than that from the summary General hepatitis,more significant difference(P<0.05).6.The results showed that:Liver depression and Qi stagnation symptoms of type 14 in 7 of the differences between 2 groups was statistically significant(P<0.05),liver and gallbladder damp-heat in 6 of the 13 symptoms between the 2 groups was statistically significant(P<0.05),cardiac-type thermal subsidence of 13 symptoms in eight difference between 2 groups was statistically significant(P<0.05),yin and yang from the 12 summary-type symptoms in the seven differences between 2 groups was statistically significant(P<0.05).ConclusionIn this study,using a variety of statistical methods,summarized hepatitis syndrome differentiation rule.Based on clinical data and analysis of the following information:①severe hepatitis was divided into four permit type,stagnation of liver qi stagnation,liver and gallbladder damp-heat,thermal subsidence from the pericardium and of Yin and Yang must type;②severe hepatitis in the medium and large part of the main chronic severe hepatitis;③thermal subsidence from the pericardium and of Yin and Yang must type with stagnation of liver qi and liver and gallbladder damp-heat in the PTA,TBIL,ALB rather typical;④Liver depression and Qi stagnation and to improve the rate of liver and gallbladder damp-heat higher than the other two types,thermal subsidence from the pericardium and of Yin and Yang must type no more efficient than the other two types;⑤hepatitis general stagnation of liver Qi stagnation were,and severe hepatitis were mainly liver and gallbladder damp-heatthermal subsidence pericardium and yin and yang from the summary-type;⑥Liver depression and Qi stagnation type 7 symptoms and signs more than the typical general hepatitis, liver and gallbladder damp-heat in 6 as compared with ordinary symptoms and signs more typical of hepatitis,thermal subsidence type pericardial 8 symptoms and signs of hepatitis as compared with ordinary more typically,from the summary of yin and yang type 7 symptoms and signs more than the typical general hepatitis.
