Diabetic peripheropathy(DPN) is a series of pathological changes happened in various nervous system of diabetic patients.Its symptoms include alternative cold and heat, burning pain,paresthesia and hyperesthesia,all of which is inclined to aggravate during night time.The signs of DPN consist of decreased sense of pain,temperature and vibration,disuse atrophy of small muscles,hypohidrosis and varicosity of instep vein.Diabetes is a chronic disease without obvious symptoms,which always lacks timely treatment and leads to multiple chronic complications.DPN,the most common complications of diabetes,is difficult to cure once occurred.Previous study of traditional Chinese medicine(TCM)has demonstrated that warming Yang to restore normal collateral principle can treat DPN effectively.The aim of the present study is to compare the effects of Chinese traditional medicine and Western medicine and to find an effective and easy way to treat DPN.ObjectThe aim of the present study is to observe the effect of warming Yang to restore normal collateral principle on diabetic peripheropathy and to explore the underlying mechanisms. Materias and Methods1.Nineteen male and fifteen female DPN patients aged between 41 to 63 years old were selected for clinical observation in the present study.All the patients were diagnosed as Flaccidity card in Traditional Chinese Medicine for at least 3 years.The patients were randomly divided into the Chinese traditional medicine group(n=16)and Western medicine group(Control,n=18).No obvious difference between the two groups were observed in sex,age,diabetic course and the control of basic level of blood sugar and blood lipid.2.While keeping previous therapy,such as diet control, exercise,blood glucose control and blood lipid control, patients of the two group were treated as below:1) Chinese traditional medicine groupThe composing prescriptions of this group were based on Processed Astragalus,Angelica Sinensis,Radix Rehmanniae, rhizomaligusticichuanxiong,milettia reticulate,Coptis chinensis,cassia twig,asarum,achyranthes root,common fenugreek,and White peony Root and were modified accordingly during the course of treatmen.Usage:One dose of the above herbs were decoted with 150 ml water and were taken orally after breakfast and supper every day;the herb residue were decoted with appropriate water for another 30 minutes to immerse feet for 25 to 30 minutes(water temperature 40~50℃,once daily).2) Control groupThe DPN patients of control group were treated with VitB1 20 mg and Adenosine Coenzyme B12 three times daily. Both groups were observed for 4 weeks.The effects of Chinese traditional medicine(CTM)and Western medicine(WM) on the nerve conduction were shown as below:ResultsSignificant effects of treatment on nerve conduction between pre-and post-treatment were observed in both CTM group(P<0.05)and WM group(P<0.01).In addition,obvious difference of Chinese traditional medicine and Western medicine was also observed,indicating that the effects of the Chinese herbs and western medicine were different.ConclusionDiabetic peripheropathy is one of the most common complications of diabetic disease.It is difficult to treat and seriously affects the daily life of DPN patients.As DPN originates from diabetes,it belongs to the syndrome of "Yang Deficiency and Blood Stasis".We treated it by means of warming Yang to restore normal collateral principle,choosing Yang supplementing Five Returning Decoction minus rhizomaligusticichuanxiong,Coptis chinensis,cassia twig,asarum,achyranthes root,and common fenugreek.It has been proved that Chinese medicine has significant effects on repairing injured nerve and improving neurotrophic microenvironments.Western medicine treats DPN patients mostly by VitB1 and VitB12,but the effect is not always satisfying.The present study compares the effects between Chinese traditional medicine and Western medicine on DPN patients.The results demonstrated that Chinese traditional medicine has significant effect on treating DPN.
