Objective To explore the treatment and prognosis of multiple intracranial aneurysms (MIA).Methods Of 76 patients with MIA,45 were treated by microsurgery and 21 were treated by endovascular embolization.Of 45 patients undergoing microsurgery,34 underwent surgery only once and 11 twice.Of 19 patients undergoing endovascular embolization,19 were treated by one - stage procedure and 2 twice.8 were treated with coiling and clipping combined,and 2 refused any treatment.Results According to GOS,59 were recovered well,9 modertely disabled,4 severely disabled,2 died.Of all treated aneurysms,9 had neck remnants after microsurgically clipping,while 3 were partially coiled.Conclusions Although MRA and 3D-CTA are widely applied in clinic in recent years, 3D-DSA is still a gold standard to diagnose MIA.MIA can be treated by microsurgically clipping or endovascular embolization or both combined.The proper choices should be selected according to the patients' Hunt-Hess grade and the distribution as well as shapes of aneurysms,There is no significant difference in outcome between microsurgery and endovascular embolization in short-term observation.
