| Objective:To observe and evaluate the effect and safety of the treatment combining Finasteride with Simvastatin onâ…¡andâ…¢degree of acne, as well as their influnence on the serum indexes of corresponding hormones,and to explore a kind of economical and safe treatment for acne.Method: All of the 96 patients of acne were collected from the special course of study acne clinic service of Changhai Hospital of Shanghai. According to Pillsbury classification, they were measured up with the diagnostic criteria of acne and were classified into gradeâ…¡andâ…¢. The patients were aged from 18 to 40.The 96 patients were randomly divided into three groups. Group A took Tanshinone,1g, tid;group B took Finasteride, 5 mg, qd;group C took Finasteride,5 mg, qd,simvastatin 20mg,qd. Each group was consisted of 32 cases. The course of treatment was 9 weeks. We had compared the decline in the number of skin rashes, curative exponent about skin rashes, and the incidence of side effects. Another ten healthy students without acne were collected as negative group, and compared the lever of some serum sex hormone indexes with which of acne group, and acquired evaluation of efficacy and safety, and compared which of acne patients of three groups after treated 3, 6, 9 weeks. All the participants had the right to informed consent.Results and discussion: 1,There is no significant deviation of ages, genders, course of disease, as well as severity of skin destruction between three groups (P>0.05). With the treatment, inflammatory and non-inflammatory lesion count of three groups decreased. After treated 3 , 6 ,9 weeks and before treatment ,every two time points comparison of the various types of lesions are statistical significance(P<0.05). We can easily suppose that the efficiency of the group C on non-inflammatory lesion count is higher than which of group A and it is statistical contrast (P <0.05)2,There is no significant difference in some serum indexes between acne group and normal groupin some serum indexes ,which including testosterone(T), free testosterone(FT), fasting serum insulin(FINS) and dihydrotestosterone(DHT)(P>0.05),but the sex hormone-binding globulin(SHBG)lever of the acne group is statistical lower than which of the normal group(P<0.05).3,The FSH lever of the three groups after treatment is highly higher than which of them before treatment. (P<0.01),and there is no significant difference in which among the three groups(P>0.05); The LH lever of the groupA and groupB after treatment is higher than which of them before treatment(P<0.05),and there is no significant difference between the two groups(P>0.05), and the LH lever of the groupC after treatment is significant lower than which of them before treatment(P<0.05); There is no significant difference between LH/FSH ratio of groupA before treatment and which after treatment,and LH/FSH ratio of groupB after treatment is highly higher than which of them before treatment(P<0.01),and LH/FSH ratio of groupC after treatment is highly lower than which of them before treatment(P<0.01);The T lever of the groupA and groupC after treatment is highly lower than which of them before treatment(P<0.01),and there is no significant difference between the two groups(P>0.05),The T lever of the groupB after treatment is highly higher than which of them before treatment(P<0.01);The FINS lever of the groupA and groupC after treatment is highly lower than which of them before treatment(P<0.01),and there is no significant difference between the two groups(P>0.05),The T lever of the groupB after treatment is highly higher than which of them before treatment(P<0.01);The HOMA-IR of the three groups after treatment is lower than which of them before treatment,and there is no significant difference in groupB(P>0.05),but there is highly significant difference in groupA and groupC(P<0.05),and there is no significant difference among the three groups(P>0.05);The DHT lever of the three groups after treatment is lower than which of them before treatment(P<0.05),and there is no significant difference in the change DHT lever between groupA and groupB(P>0.05),but there is highly significant difference in groupB and groupC(P>0.05),and there is no significant difference among the three groups(P>0.05);The DHT lever of the three groups after treatment is lower than which of them before treatment(P<0.05), The SHBG lever of the groupA after treatment is highly lower than which of them before treatmen(tP<0.01),and The SHBG lever of the groupB and groupC after treatment is highly higher than which of them before treatment(P<0.01),and there is highly significant difference among the three groups(P<0.01),there is no significant difference in groupB and groupC(P>0.05);The FT lever of the groupA after treatment is highly lower than which of them before treatment(P<0.01),The FT lever of the groupB and groupC after treatment is highly higher than which of them before treatment(P<0.01),and there is no significant difference among the three groups(P>0.05)4,Four patients clinical adverse reaction in groupA, 3 women showed a slight stomach discomfort after medication, the performance of a male for headache, drowsiness..Three patients clinical adverse reaction in groupB,the symptoms are lightly skin and itching. There isn't adverse in group C. We didn't treat these patients.Conclusion:After treated with both Finasteride and Simvastatin ,the clincical signs and symptoms of acne patients were evidently improved,and the efficiency of combination therapy was significant higher than which of therapy with Tanshinone or Finasteride alone, with no adverse side effects observed in the process of the combination therapy. At the same time, the combination therapy can obviously improve insulin sensitivity, decrease the DHT lever and LH/FSH ratio,and therefore improve endocrine function in acne patients. In conclusion, the combination therapy is a kind of economical and safe treatment for acne.
