Adriamycin is a broad-spectrum anthracycline and anti-cancer therapeutic, and has obvious neurotoxic.In recent years,retrograde adriamycin sensory ganglionnectomy has been used in treatment of trigeminal neuralgia and has obtained good therapeutic effect.The damage of adriamycin was highly selective and self-limited.Adriamycin shows great potentiality in treatment of trigeminal neuralgia and some other neuromuscular diseases.PART ONE The autofluorescence in the trigeminal ganglion after injected the compound solution of hypertonic saline and adriamycin to the infraorbital foramen of ratsObjective The purpose of this study was to observe the autofluorescence and its phase changes of adriamycin in the epineurium, infraorbital nerve and trigeminal ganglion after injected with the hypertonic saline in a compound solution from the rat infraorbital foramen. Methods 10ul compound solution of 10% NaCl and 1% adriamycin or 10% NaCl alone were injected from the infraorbital foramen. The intensity of autofluorescence in the epineurium, infraorbital nerve and trigeminal ganglion was qualitatively observed at six different time points of 10, 20, 48, 96hours, 7days and 10days after injections. The sensation of the vibrissae pad was tested every day after administration. Results The epineurium showed a continuously intensive autofluorescence of adriamycin at 10, 20, 48, 96hours after injection of the compound solution, while the autofluorescence observed in infraorbital nerve stem appeared from 10 hours after injection. The autofluorescence of adriamycin in trigeminal ganglion was first showed at 20 hours,and then reached the highest at 48 hours. The distribution of the fluorescence in trigeminal ganglia was limited and restricted to the anterolateral region. At 96hours, 7days and 10days, the intensity of autofluorescence continually subsided and disappeared eventually. There was no fluorescence neither in hypertonic saline injection group nor in the blank control group, which was independent with the time. Conclusion Adriamycin could permeate through the epineurium easily after injected together with the hypertonic saline from the infraorbital foramen and then could be transported retrograde to the trigeminal ganglia, which could probably provide a novel and effective treatment on trigeminal neuralgia.PART TWO Pathological changes of trigeminal ganglion cells after injections with adriamycin in rats by three different methodsObjective Three different methods of injection with adriamycin were performed in SD rats to compare the changes in function of trigeminal nerve and pathomorphology of trigeminal ganglion cells in different time points.To investigate feasibility that perineural (infraorbital hole)injection with mixed solution of hypertonic saline and adriamycin treat trigeminal neuralgia. Methods At this experiment,96 SD rats were randomly allocated into four groups(A,B,C,D group),and each group has 24 rats. Each group was divided into four time points: 1-week subgroup (A1 ~ D1 subgroup), 2-week subgroup (A2 ~ D2 subgroup), one month subgroup (A3 ~ D3 subgroup), 3 month subgroup (A4 ~ D4 subgroup) subjects, each subgroup has six rats. 10ul compound solution of 10% NaCl and 1% adriamycin(A group) or solution of physiological saline and 1% adriamycin(C group) or physiological saline (D group) alone were injected from the infraorbital foramen. 10ul solution of physiological saline and 1% adriamycin (B group) were injected into the infraorbital nerve stem.At 1th week, 2th week, 1th month, 3th month after operation, the remaining rats'sensation of the vibrissae of pad was tested,and the gasseringganglions were took out to observe the total number of ganglions cells, the ratio of abnormal ganglions cells and cell's appearance. Results To test sensory function of each group of animals, there was no significant difference that intercomparison between the same time subgroups of A and B group (P> 0.05) .The same time subgroups of A, B group and C, D group were separately compared, there is significant difference (P <0.01).The ratio of abnormal ganglion cells was more,and the total number of ganglions cells was less in A2,A3,A4 group than A1 group after 10ul compound solution of 10% NaCl and 1% adriamycin were injected from the infraorbital foramen(P<0.01).There was significant different amony A2,A3,A4 group(P<0.01). There was no significant difference amony the same time subgroups of A, B groups.Conclusion After 10ul compound solution of 10% NaCl and 1% adriamycin are injected from the infraorbital foramen of rats, hypertonic saline may have effect of enhancing axoplasmic transport of adriamycin, and cause necrosis of trigeminal ganglion cells. Compound solution of 10% NaCl and 1% adriamycin were injected from the infraorbital foramen can produce the same effect as injection of nerve with adriamycin, and efficacy of this method is superior to inject with physiological saline and 1% adriamycin from the infraorbital foramen.
