Objective:To investigate the serum high-sensitivity C-reactive protein(hs-CRP) level,the volume and distribution of body composition,β-cell function and the relationship between them in the obese females with different glucose metabolism.Methods:A total of 72 females were selected with strict criteria to prevent confounders.All selected objects were adopted by the oral glucose tolerance test(OGTT),divided into a normal group(NC),an obese group with normal glucose tolerance(NGT),an obese group with impaired glucose tolerance(IGT) and an obese group with type 2 diabets newly diagnosed(DM).The hs-CRP was detected by immunoturbidimetry,the body composition was measured by dual-energy X-ray absorptiometry(DEXA),and the intravenous glucose tolerance test(IVGTT)was performed.Insulin area under the curve(AUCins),acute insulin response(AIR) and homeostasis model assessmentβ-cell(HOMA2-%B)were used to assess theβ-cell function.Homeostasis model insulin resistance index(HOMA2-IR)and insulin sensitivity index(HOMA2-%S)were calculated to assess insulin resistance.Results:1.In IVGTT,the maximum insulin,AUCins and AIR in NGT group were higher than them in NC group(P<0.01),but they were declined gradually in the NGT-IGT-DM groups(P<0.01).2.Acute insulin secretion was negatively correlated with age,fasting plasma glucose(FPG),2h plasma glucose(2hPG)and HOMA2-%S,and positively correlated with total body fat,trunk fat and HOMA2-IR(P<0.05).After adjustment of age,FPG and 2hPG,it was still positively correlated with total body fat and trunk fat.In multiple regression analysis,age,FPG and 2hPG and trunk fat content were into the equation of AIR finally(r=0.724,r2=0.525).3.Hs-CRP in NGT group was higher than it in NC group(P<0.01),and higher in DM group than it in NGT and IGT groups(P<0.01).4.Hs-CRP was positive1y correlated with FPG and 2hPG, triglyceri- des(TG),total body fat,trunk fat and HOMA2-IR,and negatively correlated with HOMA2-%S(P<0.05).After adjustment of FPG,2hPG and TG,it was still positively correlated with total body fat and trunk fat(P<0.05).After adjustment of BMI,WC and TG,it was negatively correlated with AIR(P<0.05).In multiple regression analysis,2hPG,TG and trunk fat content were into the equation of hs-CRP finally(r=0.736,r2=0.542),but AIR not.Conclusion:1.In the NGT-IGT-DM process,theβ-cell function was declined gradually in the obese females.Age,obesity,plasma glucose,insulin resistance and insulin sensitivity were related toβ-cell function.Compared to BMI and WC,body composition was a more valid method for predicting earlyβ-cell function damage.2.The level of hs-CRP in the obese females was higher than normal people,specially in those with diabetes.Hs-CRP was related to obesity,plasma glucose,lipids,insulin resistance and insulin sensitivity,but not related to acute insulin secretion.Compared to total body fat,the accumulation of abdominal fat was more important for hs-CRP.
