Part I Influence of Cardiovascular Risk Factors on Plasma Fibrinogen in Patients with Essential HypertensionObjective: To explore the association between plasma fibrinogen(FIB) and cardiovascular risk factors in patients with essential hypertension(EH).Methods: A total of 1300 EH patients were divided into four group according to the number of cardiovascular risk factor:EH without caridovascular risk factor group (EH+0,n=196),EH with one cardiovascular risk factor group (EH+1,n=442),EH with two cardiovascular risk factors group (EH+2,n=433),EH with more than three cardiovascular risk factors group(EH+≥3, n=229).plasma FIB level was measured by the clotting method of Clauss.Results:1)The plasma FIB level in EH+0 group was significant lower than that in other groups(3.17±0.89 g/L vs 3.37±0.93 g/L,3.48±0.96 g/L and 3.84±1.13g/L, P<0.05),EH+≥3 group was significant higher than that in EH+1 or EH+2 group(P<0.05).2)The plasma FIB level in EH+MS group was significant higher than that in EH+nonMS group(3.59±1.01 g/L vs 3.32±0.98g/L,P<0.01).3)After adjusting for other influencing factors,the plasma FIB level was no significant difference among three grade EH(FIB:3.41 g/L, 3.42 g/L and 3.52g/L,P>0.05).Conclusion: The plasma FIB level is closely related with cardiovascular risk factors but not the blood pressure in EH. Part II The Relationship between plasma Fibrinogen,Uric Acid and the Coronary heart DiseaseObjective:To study the relationship between plasma fibrinogen(FIB), uric acid and the coronary heart disease(CHD).Methods:348 patients with or without coronary heart disease were divided into CHD group and control group. All subjects were examined by cronary artery angiography. The level of plasma fibrinogen and uric acid were compared between two groups. With the multiple analysis method, the relationships were studied between plasma fibrinogen, uric acid and coronary heart disease.Results:1)The plasma fibrinogen and uric acid level in CHD group was significant higher than that in control group (2.80±0.77g/L vs 3.68±1.09 g/L, P<0.05; 358.68±95.09μmol /L vs 327.43±88.86μmol /L, P<0.05).2)With multiple logistic regression, low-density lipoprotein , fibrinogen(OR=3.04, 95%CI:1.94~4.77, P<0.05), smoking and age were independently associated with coronary heart disease. No assiciation was found between gender, uric acid , other variables with coronary heart disease.3)After adjusting for other influencing factors, the odds ratio of hyperfibrinogenemia in CHD group obviously elevated(OR=4.97,95%CI:2.27~12.18,P<0.01),but not hyperuricemia(OR=1.36,95%CI:0.66~2.77,P=0.41).Conclusion:High plasma fibrinogen level may be an independent risk factor for CHD but not the uric Acid.
