Objective To explore the association of adiponectin level, gene polymorphism in promoter region and albuminuria in patients with type 2 diabetes mellitus of Fujian.Methods 139 patients with type 2 diabetes mellitus (T2DM), including 50 patients with normalalbuminuria (NAU group), 50 patients with microalbuminuria (MiAU group) and 39 patients with macrolalbuminuria (MaAU group), were enrolled in this study. Plasma adiponectin level (APN), fasting plasma glucose (FPG), fasting insulin (FINS), glycosylated hemoglobin (HbA1C), total cholesterol (TC), triglyceride (TG), high density lipoproteins (HDL-C), serum creatinine (Scr), body height, weight, waist circumference (WC), hip circumference (HC), systolic blood pressure (SBP), diastolic blood pressure (DBP), carotid artery intima media thickness (CIMT), were measured. Low density lipoprotein (LDL-C), body mass index (BMI), waist to hip ratio (WHR), endogenous creatinine clearance rate (Ccr), homeostasis model assessment of insulin resistance (HOMA-IR) were calculated. The adiponectin gene SNP-11377C→G polymorphism was identified by polymerase chain reaction restriction fragment length polymorphism (PCR-RFLP). The Hardy-Weinberg equilibrium was tested by Chi-square test of goodness of fit. and polymorphic frequencies were compared in patients within NAU, MiAU and MaAU groups. T test and Chi-square test was used to compare differences among groups. The association of adiponectin and other factors was test by stepwise multiple regression analysis.Results (1) Plasma adiponectin level (mg/L) showed an increasing tendency in groups of NAU, MiAU and MaAU (7.75±3.62, 7.82±3.82, 10.03±3.74). Plasma adiponectin level was significant higher in MaAU group than NAU and MiAU groups (P<0.05). No significant difference between NAU and MiAU groups in plasma adiponectin level (P>0.05). (2) Simple correlation analysis proved that plasma adiponectin level was negative correlated with CIMT (r=-0.288, P<0.05), Ccr (r=-0.250, P<0.05), HOMA-IR (r=-0.316, P<0.01), BMI (r=-0.251, P<0.05). Stepwise multiple regression analysis proved that plasma adiponectin level was negative correlated with CIMT, Ccr, HOMA-IR and BMI. (3) The frequencies of adiponectin gene genotype and allele showed no significant difference among groups of NAU, MiAU and MaAU.(4) Plasma adiponectin level showed no significant difference among groups of CC and CG+GG(P>0.05).Conclusion Aggravation of albuminuria accompanied increasing adiponectin level in in patients with type 2 diabetes mellitus of Fujian. There was no significant association of SNP-11377C→G polymorphism in promoter region of adiponectin gene and albuminuria in patients with type 2 diabetes mellitus. |
