Choice Of Prosthesis In Hip Replacement For Elderly Patients With Femoral Neck Fracture

【Objective】Analysis of the two kinds of surgical treatment for the elderly patients with femoral neck fractures :Artificial femoral head replacement(FHR)and Total hip arthroplasty(THA),Compariing of the above-mentioned advantages and disadvantages of two kinds of surgery , especially for different ages of elderly patients with femoral neck fractures so that we can make a more reasonable choice of artificial hip prosthesis for those patients。【Study method】1.From December 2006 to Jan 2009,During this 2 and a half years,our hospital admitted 57 cases of elderly patients with femoral neck fracture。At the age of 60-95 years old,The average age is 72-year-old, A group is form 60-year-old to 70-year-old which including17samples ,9 samples got FHR,8 samples got THA.B group is form 71-year-old to 80-year-old which including 24 samples,13 samples got FHR,11 samples got THA .C group is form 81-year-old to 95-year-old which including 16 samples.8 samples got FHR,8 samples got THA .2.the standard for the Samples :①. According to preoperative medical history and conventional test results, When patients admitted to hospital and was taken X-ray of bilateral hip which can diagnose femoral neck fracture。②.Age greater than 60-year-old。Exclusion criteria :①.Rule out the possibility of unrelated death,the loss of follow-up or incomplete data②. Rule out femoral neck fracture which do not belong to the first type of femoral neck fracture and by neck type。3.Three groups of patients had complete follow-up data。Their surgery and postoperative corresponding assessment were recorded。the project of assess to the 57 patients including Length of stay, Operative time,Bleeding volume,Activity time after operation;Systemic complications includes infections, fractures, deep venous thrombosis, vascular injury, nerve injury, short-term renovation of surgery, postoperative DIC,stress ulcer, and other infections, such as life-support systems。local complications include pain, length of both lower extremities, hip joint space changes, prosthetic dislocation, loosening of prosthesis subsidence , The patient's pain of three ground had been taken an individual assessment , The follow-up was mainly taken patients to hospital for diagnosis and clinical examination,Periodic review of bilateral hip and other films,etc。4.Statistical analysisBy using the software SPSS13.0 to deal with the data collected from all patients ,using mean±standard deviation to denote The data。Using t test for the Measurement data。 using chi-square test for the Count data。P value <0.05 means the differences have significant。【Study result】All patients had regular follow-up。There were no postoperative deaths in patients,with no infection, fracture, vascular injury, nerve injury, the center dislocation of acetabular complications,Etc。Systemic complications:There is not any Systemic complications in A group。B group FHR includes 3 samples,THA includs 5 samples; C group FHR includs 3 samples,THA includs 7 samples;local complications:A group FHR includs 7 samples,THA includs 2 samples; B group FHR includs 4 samples,THA includs 2 samples;C group FHR includs 4 samples,THA includs 1 samples.pains:A group FHR includs 7 samples,THA includs 2 samples; B group FHR includs 7samples,THA includs 3samples;C group FHR includs 3 samples,THA includs 2 samples.
