Objective:To improve the diagnosis and treatment of adrenal incidentalomas.Methods:A retrospective analysis of the clinical data of 73 patients with adrenal incidentalomas between 2004.6 and 2008.6.Results:Adrenal incidentalomas in this group high age was 30 ~ 60 years old, healthy adrenal incidentalomas are found in the main channels. 73 cases in this group, 63 cases of benign tumors (86.30%), 10 cases of malignant tumors (13.70%); 62 cases of non-functioning tumors (84.93%), 11 cases of functional tumors (15.07%). B-, CT, MRI diagnosis of the position were in line with the rate of 85.92%, 90.91% and 92.86%, in line with the rate of diagnosis of 45.07%, 74.24%, 85.71%, B Chao, CT and MRI diagnosis in line with the rate (P> 0.50); B super diagnosis was significantly lower than that in line with the rate of CT or MRI (P <0.005), CT and MRI in line with the rate of diagnosis (P> 0.25). Line in this group are the treatment of adrenal tumors, 43 cases of laparoscopic surgery (58.90%), open surgical treatment of 30 cases (41.10%). Benign tumor of the average diameter of 3.68cm (1.1 ~ 9.1cm), the average diameter of malignant 7.83cm (3.6 ~ 14.5cm), benign tumor diameter significantly smaller than malignant tumors (P <0.01). 4.3cm tumor was benign and malignant tumors of the best cut-off point (≥4.3cm malignant), a sensitivity of 90% and specificity of 72.5%, Youden index to obtain the maximum value (0.625), Kappa value of 0.435 (P <0.01) with the pathological diagnosis consistent with a high rate.Conclusions: 1. Benign adrenal incidentalomas in the majority of non-functioning tumors, some malignant tumors and functions as a tumor. 2. B-ultrasonography, CT and MRI diagnostic value of the positioning, CT and MRI is superior to the qualitative diagnosis of B-ultrasono- graphy.3.The diameter of adrenal incidentalomas are benign and malignant tumors to determine the reference indicators of benign and malignant tumor of 4.3cm as the cut-off point (≥4.3cm malignant); for malignant tumors, functional tumors and tumor diameter greater than 4.3cm, Surgical treatment should be taken; of 4.3cm less than the benign non-functioning tumors may choose to observe and periodic review of a conservative.
