The present study was actually aimed at providing scientific reference for making health development plan about the new model of rural cooperative medical service, insuring the safety of running fund and protecting the benefit of rural paticents. The whole programe was performed by literature review, questionnaire and interviews to understand the whole hospitalization cost status at different levels of hospitals, especially those engaging in this medical service system.And then of hospitalization cost , information of seeking medical advice in the different levels of hospital and the factors influencing the hospitalization cost , which were analyled and compared each other by the softwares of Epi-Data 3.02, SAS 8.12 and SPSS15.0 .The problems and controllable factors influencing the hospitalization cost were looked for.(1) The whole status of new model of rural cooperative medical in Hubei province was good and advanced, providing a good environment for the hospitals of differential levels taking part in the rural cooperative medical service.(2) The hospitalization cost in the villages and township hospitals for the patients and afforded by the patients under this system; The villages and township hospitals could not satisfy the needs of the patients in the cooperative medical service,because of their poor basical building and backward techniqus and equipment. The rational distribution of patients at different levels was not realized by economical adjustaent.(3) The irrational cost in the hospitals constitution was as follows: 1.the cost of diaghosis and treatinent technigues was still low. Cost of drugs and examinations with eguipments was high, controllable and could be lowered. The status in the hospitals at different levels was as follows. The villages and township hospitals ran badly.The county hospital ran at middle level, the trend of their development was bad and the cost of drugs and examination was irretional, the above-county hospitals ran well, but the cost of drugs and examination was relatively irrational.It was suggested that the hospitalization cost was closely related to the control of cost under new rural cooperative medical service. 3. Sugery cost was mainly influenced by the introduction of new techniques and materials, of which choices by the surgeous and patients were sigifically related to the hospitalization cost. The traditional sugery application guildlines were out of use already.The ideas which costral hospitcalization cost are proposed as follows:(1) The strict accessment of the hospitals which go into the cooperative medical service system should be performed. An effective and scientific accessment system matching this medical service system should be estalished.(2) That the hospitals join the cooperative medical service should be encouraged, for control of hostitalization cost and perfecting the medical service.(3) The cost of in drugs and examinations, with eguipacal should be at same time, the running level of the county-above hospitals should be decreased to avoid the effect of the placed under striatly serveilance bad trend on villages and town hospitals.(4) Macroscopically, the villages and town hospitals should besupported by policy and financial resources to perfect of basical building, equipments and techniques; micro scopically, the communication and cooperation between the hospitals,at different levels forming a natural referring system running well.(5) Differnt running models of this medical system in the different areas should be standardized and unitied step by step. In this way, a total and correct accessment could then be conducted, and the rational political suggestions, and solvement of rural patients'medical service problems may be oblained.
