Objective: To approach the relationship between clinical pathofeatures and metastasis in conglobate gland, to predict the tendency of metastasis in conglobate gland, to provide the clinician with reference to clinical therapy.Methods: In order to investigate the relation of clinical patho-characters and lymphatic metastasis in the colorectal cancer, we made a retrospective study, 1489 cases which were colorectal cancer evidenced with comparatively complete pathology after the operation were selected in the Fourth Hospital of Hebei Medical University from Jan 1998 to Dec 2006. All data collected was set up EXCEL database. SPSS statistics software wase applied. Thenχ~2-test wase used to analyze the correlation factor separately, Logistic regression analysis wase used to analyze multiple factors (a=0.05 for significance level, partition of chi-square wase employed , if necessary).Results: The lymphatic metastasis positive rates of different sex and curse were not statistically different. The lymphatic metastasis positive rate of youngster(≤35) was 60.00%, which was higher than the rate of the middle-aged (36~59) and old group(≥60)(18.61%,16.25%). The lymphatic metastasis positive rates of youngster group and middle-age or old group were significantly statistically different(χ~2=12.797, P<0.01;χ~2=17.753, P<0.0125), but the rates of middle-age and old group were not statistically different (χ~2=1.589,P=0.207). The lymphatic metastasis positive rate of rectal cancer (42.46%) was slightly higher than the rate of colon carcinoma (35.13%), which has statistical significance (χ~2=5.531,P=0.019). In addition, the lymphatic metastasis positive rate increased, if the tumor wase closer with anocutaneous line. However, it wase not statistical difference. According to the DA(≤3cm, >3~<6cm and≥6cm ) of the tumor, the cases were divided into three groups, The lymphatic metastasis positive rate of colorectal cancer increased (35.91%,39.77%,46.80%),and there wase statistical significance difference (χ~2=10.408, P=0.005). Another way these cases were also divided into three groups according to differentian, the well differentiated group (well differentiated adenocarcinoma, papillary carcinoma and adenoma canceration), the moderately differentiated group (moderately differentiated adenocarcinoma and SC ) , the poorly differentiated group (poorly differentiated adenocarcinoma, mucinous adenocarcinoma signetring cell carcinoma, and undifferentiated carcinoma). The poorly differentiated group (62.22%) wase easier to metabasis than the moderately and well differentiated group (38.18%, 20.09%), these three groups were statistically different (χ~2=39.878, P=0.000). but there wase not statistically difference between the moderately and well differentiated group (χ~2=1.403, P=0.236). The lymphatic metastasis positive rates were statistically different, along with different depth of enteric cavity, which the tumour cell infiltrated. The rate of lymphatic metastasis involving full-thickness or over-thickness was 46.88%, and the rate of involving muscular layer was 22.59%, the rate of limited to tunica mucosa or tunica submucosa wase 0%, There wase statistically difference among three groups (χ~2=82.346, P=0.000). And the lymphatic metastasis positive rate of rectal cancer wase prone to be higher, if the circumscription of rectal tumor invaded enteric cavity becomed larger. And there wase statistically difference (χ~2=17.813, P=0.003). There were many factors relating to lymphatic metastasis. So multi-factor-analysis suggests that the rate of lymphatic metastasis wase relative to the infiltrating depth, cell differentiation. And the age was ruled out. The capital factor wase the infiltrating depth.Conclusions: The patients' sex and curse are not related to the lymphatic metastasis positive rate of colorectal cancer. The age, pathologic typing, differentiation degree, infiltrating depth and circumscription are related to. And the most important factor is the infiltrating depth.
