Diagnostic Value About The Level Of CA153 And CA99 In The Malignancy-Relted Ascites

Objective:To explore the level of CA153 and CA199 in serum and ascites about malignancy- related and non- malignant ascites.Investigate the value about ascites level of CA153 and CA199 in the diagnosis and differential diagnosis of malignancy- related and non- malignant ascites.Methods:Measure the level of CA153 and CA199 in serum and ascites in malignancy- related ascites group(14 cancer of stomach,11 rectal cancer patients,8 oophoroma,6 cancer of the liver,2 carcinoma of the gall-blader,1 Cholangiocarcinoma,1 pancreatic carcinoma),and non-malignant ascites group(26 hepatocirrhosis,8 Tuberculous Peritonitis,1 Severe hepatitis).Making a comparison with 40 normal subjects.Analysis the ascites level of CA153 and CA199 in the differential diagnosis of malignancy-related and non- malignant ascites.Result:The level of CA153 in serum malignancy- related ascites was significantly higher than non- malignant ascites(t′=2.11,P＜0.01) and normal subjects(t′=3.36,P＜0.05).There were no significant differences between the groups of non- malignant ascites and normal subjects(t′=0.624,P＞0.05).The level of CA199 in serum malignancy- related ascites and non- malignant ascites were significantly higher than normal subjects(t′=3.80和3.74,P均＜0.01).The level of CA199 in serum malignancy- related ascites was higher than non- malignant ascites,but there was no significant difference(t′=1.50,P＞0.05.The level of CA153 and CA199 in ascites malignancy- related ascites were significantly higher than non-malignant ascites(t′=2.36,P＜0.05;t′=4.52,P＜0.01).The diagnostic sensitivity of serum CA153,CA19-9 in the malignancy-related ascites were 41.86%and 53.49%.And the specificity were 80.60% and72%.The diagnostic sensitivity of combined assay of serum CA153 and CA19-9 was 72.09%,specificity was61.33%,positive predictive value was 51.67%,negative predictive value was79.31%o The diagnostic sensitivity of ascites CA153,CA19-9 in the malignancy- related ascites were48.84%and53.49%,And the specificity were91.43%and 85.71%。The diagnostic sensitivity of combined assay of ascites CA153 and CA19-9 was 79.07%,specificity was 80.0%,positive predictivevalue was 82.93%,negative predictive value was 75.68%.Conclusions:Detect the level of CA153 and CA199 in serum and ascites individual was limited in the diagnosis of malignancy-related.But combined assay of ascites CA153 and CA19-9 in the differential diagnosis of malignancy-related and non- malignant ascites has certain value.
