Experimental Study On The Edema Caused By The Application Of RhBMP-2 To Anterior Cervical Interbody Fusion

Objective:To study the occurrence of postoperative edema of soft tissue caused by the application of rhBMP-2 to anterior cervical interbody fusion and discuss it's mechanism.Methods:12 goats were randomly divided into two groups,all the goats were applicated with the anterior cervical disectomy and fusion with allograft The experimental group's 1 mg rhBMP-2 was filled into disc space for the study group.The same procedure was performed for the control group without using rhBMP-2.Symptoms such as choke drinking water,tachypnea and the activities of limbs were observed to make sure if the goats had soft tissue edema postoperatively.CT was performed for each goat preoperatively and postoperatively to compare the areas of the edema between the two groups.The area of the anterior cervical soft tissue edema was measured using ImageJ software on CT images.All the experimental goats were sacrificed immediately after CT examination,surgical anatomy was carried out to the corresponding segment of organizations,to observe the corresponding parts of the operation,in particular,the corresponding edema changes in soft tissue,and took department of trachea,esophagus and neck muscle length having the most obvious edema pathology and immunohistochemistry experiments.Results:Two days before the operation,the experimental group's time of beginning to ruminate was 49.04±3.8 minutes,the control group was 47.21±5.61minutes;ruminant duration of experimental group was 35.04±3.21 minutes,the control group was 33.31±4.68 minutes;ruminant number within 24 hours for the experimental group was 5.34±0.86times,the control group was 6.11±1.21 times;respiratory rate per minute for the experimental group was 12.35±1.78 times,the control group was 11.11±2.29 times,there was no statistic significance within the two groups;Experimental group's time of beginning to ruminate was 74.00±4.60 minutes,the control group was 60.17±6.05 minutes;ruminant duration of experimental group was 16.00±3.57 minutes,the control group was 25.17±5.00 minutes;ruminant number within 24 hours for the experimental group was 1.67±0.82 times,the control group was 3.17±1.17 times;respiratory rate per minute for the experimental group was 23.83±4.07 times,the control group was 16.00± 2.90 times;CT imaging experiment shows that the area of edema of the experimental group was 305.67±29.92 mm~2,the control group was 275.00±12.25 mm~2;there were obvious local inflammatory effusion in the experimental group,no in the control group. Histological results on the existence of HE edema,the experimental group edema goats have to be significantly heavier than the control group,immunohistochemical characterization of the results of the experimental group and control group and the cytoplasm of stromal cells in both rhBMP-2 expression,edema could be seen on the experimental group and control group results by the results of HE,experimental group's edema was also severer than the control group's,edema,qualitative immunohistochemical results of the experimental group and control group showed the rhBMP-2 expression in the cell cytoplasm and the intercellular substance, immunohistochemistry quantitative results showed that the experimental group's average absorbance for neck long muscular was 0.2181±0.0256,the control group was 0.0949±0.0222;experimental group's average absorbance for trachea was 0.1935±0.0121, control group was 0.0866±0.0142;experimental group's average absorbance for esophagus was 0.1884±0.0275,control group was 0.1015±0.0133.All the data statistical detection between experimental group and control group was P<0.05,all the statistic has significance.Conclusion:This study shows that rhBMP-2 application can make the anterior cervical tissue including trachea,esophagus and neck muscle length have severer edema.We think the reason is rhBMP-2' immersion to the cervical soft tissue and the related immune response.
