BackgroundAmyotrophic lateral sclerosis which selectively impinges superior and junior motor nerve cells is a emanatory,chronic and fatal neurodegenerative disease.The mostly clinical representations of ALS which badly restrict QOL and pulmonary function include myasthenia,amyotrophia and fascicle vibration, furthermore,deglutition embarrassment,drinking choke,anarthria and hard breath are also involved.Presently there is no available therepy in Westen medicine.Lately,potential curative effects were discovered in ALS patients treated by TCM.PurposeTo explore the effect on quality of life and indexes of pulmonary function of patients with amyotrophic lateral sclerosis by strengthening the spleen and the lung,and to probe into the clinical treatment of ALS.MethodsPatients consistent with the inclusion criteria were included in the study, the ALSFRS-R Scale and ALSAQ-40 questionnare were used as quality of life rating scales and the indicators commonly used in our hospital were used to monitor the lung function,then a uniform ALS cases-observed table was constituted.Use the forward-looking research of own control before and after the treatment by strengthening the spleen and lung,collect and pack up the datas of ALSFRS-R Scale,ALSAQ-40 questionnare and indexes of pulmonary function pre-treatment,one month and three months after treatment,and collect the general data(sex,age,chief complaint,disease,etc.).Then use the SPSS13.0 statistics double to inter and check the datas,the descriptive analysis,one-way ANOVA,paired samples t test and rank sum test for statistical analysis,and observe whether or not the difference was statistically significant in the ALS patients ALSFRS-R scale,ALS self-assessment questionnaire score and lung function indicators pre-treatment,one month and three months after treatment.ResultA total of 23 cases were brought into the research.16 cases were male, seven cases were women.Onset of upper limb symptoms were in 15 cases,onset of lower limb symptoms were in 4 cases,onset of bulbar symptoms in 4 cases. Indicators of lung function were five cases of the normal range,mild restrictive dysfunction of pulmonary ventilation in 6 patients,moderate restrictive pulmonary function impairment in 8 patients,moderate pulmonary ventilation hybrid dysfunction in 1 case,severe restrictive lung ventilation three cases of dysfunction.Before treatment,one month and three month after treatment,the ALSFRS-R score between the comparison was no significant difference(P>0.05),the difference between the functional scores in the medulla oblongata,upper limb function,lower limb function and respiratory were not statistically significant(P>0.05);The difference in ALSAQ-40 score between the comparison was statistically significant(P<0.05),differences between the comparison in the social interaction and emotional response score were statistically significant(P<0.05),and the score in the ability of daily life,diet and physical exercise capacity was no difference between the comparison(P>0.05);Indicators of lung function in addition to MV,FVC and FEV1.0 which reflect the aeration function after three-month treatment were not significantly different from those before treatment.ConclusionThe treatment by strengthening the spleen and the lung,can not be considered significantly to improve the ability of the body movement, self-care ability and catering capabilities in patients with ALS.Whereas it can significantly improve their social skills and psychological effects,and the indicators MV,FVC and FEV1.0 which reflect the aeration were improved while the indicators reflecting the lung capacity,small airway function and diffusing capacity were not markadly impacted.
