Imaging Analysis Of Lumbar Spondylolisthesis And Study Of Multiple Spiral CT 3D Reconstruction In Lumbar Isthmic Spondylolisthesis

ObjectiveTo analyze the relationship between Spondylolisthesis of oblique angle of L4,L5 lumbar body and lumbosacral angle in patients with isthemic Spondylolisthesis,degenerative spondylolisthesis(DS) and healthy people.To study diagnostic value of MSCT 3D reconstruction in lumbar isthemic spondylolisthesis.Method50 isthmic pondylolysis patients(24 men and 26 women) aged from 21 to 68 years(mean,48.6years),50 degenerative spondylolisthesis patients(24 men and 26 women) aged from 48 to 67 years(mean,58.6 years),and 50 normal healthy people(26 men and 24 women)aged from 30 to 69 years(mean,51years) were randomly recruited.The radiography was taken on a Siemens 500mA digital X-ray machine.The images were sent to the system of PACS.Lumbosacral angle and oblique angles of L4,L5 lumbar body were measured.The data base was established on SPSS13.0 software.The data were analyzed with ANOVN Test.16 slices spiral Siemens MSCT and Siemens digital X-ray examination were taken in 50 patients with(or X-ray suspected of) isthmic spondylolisthesis. The images were reconstructed with multiplanar reconstruction(MPR),volume rendering technique(VRT)and maximum intensity projection(MIP).ResultsThe oblique angle of the L4,L5 lumbar body and the lumbosacral angle of isthemic spondylolisthesis patients are statistically bigger than of degenerative spondylolisthesis patients and normal person.In 50 patients,X-ray examination showed 37 cases of isthemic spondylolisthesis,while MSCT scanning and 3D reconstruction detected all of these patient correctly.MSCT also showed other complicated signs including narrowing of spinal canal or lateral recess,double margin sign,deformity of the inter-vertebral disc,denergeration of joint facet and thickening of the ligamentum flavum.Scotty dog sign was showed in images reconstructed along the direction of vertebral arch.spondylolisthesis were showed in sagittal images.ConclusionThe oblique angle of the L4,L5 lumbar body and the lumbosacral angle plays an important role in etiology of isthemic spondylolysis.Person with a bigger oblique angle is predilected to isthemic spondylolysis.MSCT and 3D reconstruction is the most valuable examination in diagnosis of isthemic spondylolisthesis,especially in X-ray suspected cases.
