Objective: To study the expression of CD147 and matrix metalloproteinase-9 (MMP-9) in breast carcinoma . Their relationships between the two factors and with clinicopathologic factors were analyzed . To investigate the effect of neoadjuvant chemotherapy (NACT) on the expression of CD147 and MMP-9 in breast carcinoma and determine whether there is a relationship between the two factors and the effect of NACT.Methods: The expression of CD147 and MMP-9 in 60 patients with breast carcinoma , 20 cases with normal breast tissues adjacent to tumor and 48 patients who received NACT before and after the treatment respectively were examined by immunohistochemical staining.Results: The positive expression rates of CD147 and MMP-9 in breast carcinoma were 63.3%(38/60),68.3%(41/60); respectively ,showing a significant difference between breast carcinoma and normal breast tissues adjacent to tumor (p<0.05). The expression of CD147 was not correlated with age, menstrual status, tumor size, PR and histological grade(p>0.05), but correlated with lymphatic metastasis,TNM staging and ER(p<0.05); No significance was seen in the rate of expression with age , menstrual status ,tumor size and PR(p>0.05), the expression of MMP-9 was significantly correlated with lymphatic metastasis, TNM staging, histological grade and ER(p<0.05). Expression of CD147 was positively correlated with expression of MMP-9(r=0.461, p<0.01).The expression of CD147 and MMP-9 in post- neoadjuvant chemotherapy[41.7%(20/48);64.6%(31/48)] was significantly lower than that in pre-neoadjuvant chemotherapy [62.5%(30/48),79.2%(38/48)] . After chemotherapy the positive rate of CD147 had a statistic significance (p<0.05), but MMP-9 had no(p>0.05). Before neoadjuvant chemotherapy, the positive group of CD147 was 30 cases while MMP-9 positive group was 27(90.9%); the negative group of CD147 was 18 cases while MMP-9 positive group was 11(61.1%). CD147 expression intensity was correlated with MMP-9(r=0.439, p<0.01). After chemotherapy, the positive group of CD147 was 20 cases while MMP-9 positive group was 18(90.9%); the negative group of CD147 was 28 cases while MMP-9 positive group was 13(46.4%). CD147 expression intensity was correlated with MMP-9(r=0.450, p<0.01).Conclusions: There are overexpressions of CD147 and MMP-9 in breast carcinoma, the expression of CD147 was correlated with lymphatic metastasis,TNM staging and ER ; while MMP-9 was significantly correlated with lymphatic metastasis, TNM staging, histological grade and ER. The expression of CD147 was positive with MMP-9, the variation of their expressions may contribute to the pathogenesis, development and prognosis of breast carcinoma. The expression of CD147 decreased significantly in patients who react to NACT, but had less effect on MMP-9. CD147 may be considered as a new predictive indicator for the therapeutic effect of NACT.
