Objective:Objective:To establish a rabbit peritonitis model caused by intestinal perforation,study the peritonitis' impairment to the main body organs liver,lung and kidney caused by perforation,and to explore treatment and treatment-related effects.Methods:TEST 1:20 healthy rabbits were randomly divided into 2 groups, each group had 10:sham-operated group(A group),intestinal perforation group(B group).Through the establishment of the control group,established peritonitis model caused by intestinal perforation,study the peritonitis' impairment to the main body organs liver,lung and kidney using blood test,blood biochemistry test,pathology and TUNEL research.TEST 2:50 healthy rabbits were randomly divided into 5 groups,each group had 10:intestinal perforation repair group(C group),imipenem treatment group(D group),ulinastatin treatment group(E group),Xuebijing treatment group(F group) and imipenem combined ulinastatin,Xuebijing treatment group(I group).Repairred the perforation and treated peritonitis with imipenem,ulinastatin and Xuebijing,To study their protective effect.Results:TEST 1:1.The blood gas examination:Sham-operated group(A group) with the perforated group(B) Comparison of blood pO2,blood pCO2 differences(P<0.05);blood lactate Lac sham group(A group) with the perforated group(B group) comparison,there was a significant difference(P<0.01);the rest of the indicators is no difference(P>0.05).2.Blood biochemical examination:Sham-operated group(A group) and the perforated group(B) Comparison of blood AST differences(P<0.05), ALT,BUN,there was a significant difference(P<0.01).3.HE staining of liver:A group had congestive hepatic sinusoid,some liver cells with mild edema,no obvious necrosis and inflammatory,cell infiltration;B group(perforated group) performanced liver sinusoids and a remarkable expansion of central venous congestive heart,liver cells were highly extensive edema,showing focal necrosis and bridging necrosis, lobular health district and within obvious infiltration of inflammat- ory cells;HE staining of kidney:giomeruli and tubules of A group was no significant abnormality; B group(perforated group):tubular mild edema,some samples can be seen piercing tubular severe edema.4.Liver TUNEL examination:In liver organizat- ion of A group presents two to perish weakly cell in low power field,may regard as the physiology perishes weakly;In liver organization of B group presents many dyeing blue perishes weakly cell in the high power field,perishes obviously quite weakly compared with A group,the low power field is more obvious.Kidney TUNEL examination:In A group of low time power field kidney organization the ceil seen rarely,in B group of low time power field,kidney organization disperses that the cell is perishing weakly in the tubulus renalis epidermis.TEST2:1.The blood gas examination:Perforated patch group compared with the piercing,blood lactic acid in the determination of the existence of differences(P<0.05),remaining there was no difference(P>0.05).Imipenem treatment group(D group) and perforation of the comparison group,in the partial pressure of oxygen, carbon dioxide partial pressure on the differences(P<0.05),remaining there was no difference(P>0.05).Ulinastatin treatment group(E group),Xuebijing treatment group(F group) and perforation of the comparison group,in the determination of the existence of blood lactic acid significantly(P<0.01),remaining there was no difference(P>0.05).Imipenem joint and ulinastatin Xuebijing the treatment group (G group) and perforation of the comparison group,there are differences in the partial pressure of oxygen(P<0.05),blood lactic acid in the determination of the existence of significant differences(P<0.01),remaining there was no difference(P>0.05).2. Blood biochemical examination:Perforation repair group(C group) group and perforation group(B group) compared to that in AST,ALT,BUN determination of the existence of differences(P<0.05).Imipenem treatment group(D group),ulinastatin treatment group(E group),Xuebijing treatment group(F group) and the perforated group(B group) compared to that in AST,ALT,measured on different(P<0.05); Xuebijing treatment group(F group) and the perforation group,the BUN is also different(P<0.05).Imipenem joint and ulinastatin Xuebijing the treatment group(G group) and the perforated group(B group) compared to that in AST,ALT,BUN determination of the existence of significant differences(P<0.01).3.HE staining of liver:treatment group:Those shown that various degrees of congestive liver sinusoids, liver cells had mild to moderate edema,liver cytoplasm were pultaceous,we can see the focal necrosis but no bridging necrosis.HE staining of kidney:tubular mild edema,some samples can be seen piercing tubular severe edema,no obvious glomerular lesions,perforation group and treatment group in each group had no significant difference between the lesions.4.Liver TUNEL examination:The C-G group(treatment group) in the liver organization disperses wildly isometric dark blue in low power field,in the high power field perishes cell quantity to be short obviously weakly in B group.Kidney TUNEL examination:the C-G group compares not obvious difference with B group,and the pathological change were all lightly, apoptosis cell and the liver organization quite obvious reduction.Conclusion:The test was successfully established intestinal perforation-induced peritonitis model,in the sub-model based on correlation detection was found that intestinal perforation-induced peritonitis led to the liver,lung and kidney damage in varying degrees.Intestinal perforation through the patch and the use of imipenem, ulinastatin,Xuebijing drugs to a certain extent reduce to the organ damage.
