| Objective:A mass of clinical and epidemical surveys showed that moderate or advanced periodntitis is related to cardiovascular diseases,especially the coronary heart diseases(CHD),but the true mechanism is unknown.To explore the relationship between periodontal disease and coronary heart disease,some CHD patients with periodontitis or without periodontitis were chosen to make an epidemic survey.Meanwhile,some of them have been taken blood sample to observe the level of high sensitivity c-reactive protein(hs-crp),fibrinogen(Fg),thromboxaneB2(TXB2) and6-Keto-prostaglandinF1α(6-K-PGF1α) by immunoturbidimetry or radioimmunoassay.Methods:This research includes two parts:1.The epidemic survey:65 CHD patients were chosen randomly referred for diagnostic coronary angiography.All the cases were performed clinical periodontal exam and serum test and were divided into two groups by the periodontal status:Agroup and Bgroup.Moreover,the data were analyzed.2.Laboratory Test:20 subjects were chosen from the individuals above after adjusted by such factors as age,sex,race,weight,height,blood glucose and blood pressure.They were divided into 2 groups,one of which suffered from periodontitis,the other was made of 10 CHD patients without periodontitis.Meanwhile,in the dental department of Tianjin general hospital,10 patients only suffering from perodontitis and 10 healthy volunteers were chosen.All of them underwent a comprehensive periodontal and serum examination.And then,blood sample were withdrawn and hs-crp,Fg,TXB2 and 6-K-PGF1α level in serum samples were detected.Results:1.The epidemic survey showed that:there were nodifference between two groups in the aspect of sex,age,Body Mass Index(BMI) and the high blood pressure history.The level of blood glucose(GLU-AC),total cholesterol(TC),triglyceride(TG),blood uric acid(BUA) and white blood cell(WBC) account of B group was higher than Agroup's.The level of high density lipoprotein was in opposite.The differences all had statistical significances.Meanwhile,Pearson correlation analysis showed that loosen teeth, sulcus bleeding index(SBI) and calculus index(CI) had significancantly positive correlations with the level of cholesterol;The number loosen teeth and CI had positive correlations with the level of triglyceride;The number of loosen teeth,deficient number of teeth,residual root and crown number,SBI and CI also had significantly positive correlations with the WBC account.2.Serum test showed that:age,sex,BMI and smoking history had no significant differences among four groups(p<0.05);One-way analysis of variance showed that the level of hs-crp,Fg,TXB2 and 6-K-PGF1α in serum from the patients including CP,C and P group was higher than H group(p<0.05).The level of hs-crp and Fg in serum from CP group's patients was higher than P group's(p<0.005);The correlation analysis showed that the level of hs-crp and Fg had positive correlations with BI and CI(P<0.05);The level of hs-crp also had positive correlations with PD(P<0.003~0.05);The level of GLU-AC,TC,TG, low density lipoprotein(LDL),high density lipoprotein(HDL) and WBC account had significant differrences(p<0.05);the level of TG,GLU-AC and WBC account in serum from the patients including CP,C and P group was higher than H group(p<0.05);The level of hs-crp and Fg in serum from CP group's was higher than C group's.(p<0.05).Conclusions:1.Periodntitis can result in the increases of the blood fat level and the more serious the inflammation was,the higher the blood fat level was.2.Periodontitis and CHD both can cause the highten of hs-crp,Fg,TXB2 and 6-K-PGF1α.3.Hs-crp,Fg may be the risk factors of CHD.4.Periodntitis could cause or promote the development of CHD by the "inflammation-CHD related risk factors-CHD"path. |
