ObjectivesTo adjust the inserting length of nasogastric tube correctly to reach the best effect for gastrointestinal decompression;To find out the most convenient measuring method through body surface to confirm the inserting length;to analyze the relationship between the height and the length of inserting nasogastric tube.MethodsAccording to the strict inclusion and exclusion criteria,Select 120 patients who had undergone the selective abdominal operation and reserved the whole stomach in Tianjin Medical University General Hospital from March,2008 to February,2009,and allocate them randomly to four groups:control group(n=30),test groupl(n=30),test group2(n=30),test group3(n=30).the control group inserted the nasogastric tube according to the traditional length.the test groupl increased by 5cm to insert up the traditional length.the test group2 increased by 10cm to insert.the test group 3 increased 15cm to insert.measure the distance through body surface and insert the nasogastric tube according to the corresponding length of different group that the patient should be decised to enter ahead of time.collect the relative data before and after surgery.Analyze data using statistic software.Results1.the distance from the front of gastric tube to cardia of stomach for every test group was longer than that of the control group for gastrointestinal decompression(p<0.05)2.there were significant differences among four groups in the term of the volume of gastric juice' drainage on the same day of surgery and the first day after surgery(p<0.05),however,there were no significant difference among four groups on the secend day after surgery(p>0.05).3.There were no significant differences among four groups in the terms of the first flatus(p>0.05),the time of stopping gastrointestinal decompression(p>0.05);there were significant differences between test group2 and the control group in the terms of the wound healing(p<0.008),the instance of abdominal distension(p<0.008).4.There were significant differences among the distances from tip of the nose to ear lobe and then to the xiphoid process,from the hairline to the xiphoid process,from the hairline to the navel,from the hairline to the midpoint of the xiphoid process and the navel,from the glabella to the navel.(p<0.05)5.There was no significant difference only between the distance from hairline to navel and the right inserting length(p>0.05),while there was significant difference between any other measuring distances and the actual inserting length(p<0.05).6.The actural inserting length was positive correlation to the height,the correlation coefficient was 0.561(p<0.05).The regression equation bewteen the inserting length ofnasogastric tube(Y) and the height(X) was Y=0.228X+25.957.Conclusions1.If inserting the tube according to the traditional length from 45cm to 55cm,there were only one or two holes of nasogastric tube in the stomach,and it could not drain the gastric juice effectively,so we should increase the inserting length adequatly.2.Insert the gasric tube inceasing by 10cm up the traditional inserting length for the gastointestinal decompression,which not only meet the requested length but also didnot bend in the stomach.3.The effect was the best when inserting the gastric tube increasing by 10cm for the gastrointestinal decompression.4.We could not be able to substitute one for another about the measuring methods through body surface when inserting the nasogastric tube.We could access the inserting length according to the distance from the hairline to the navel,which was easily measured and accordant with the actural inserting length.5.The actural inserting length of nasogastric tube was positive correlation to the heigt of the patient.the regression equation bewteen the inserting length of nasogastric tube(Y) and the height(X) wasY=0.228X+25.957.
