Small intestinal adhesion ileus is intestinal adhesions or the intestinal obstructionof abdominal cavity adhesions, one of the common complications after abdominalsurgery, the incidence of various types of intestinal obstruction in20%to40%, almost80%in small bowel obstruction. The disease is easy to attack repeatedly, difficulttreatment also increases the burden on patients with physical, mental, and economic,can also lead to severe intra-abdominal infections, poisoning and life-threateningintestinal necrosis. The main treatment of this disease is surgery and conservativetreatment, irrespective of treatment as soon as possible to reduce intestinal pressure isthe key of the treatment of intestinal obstruction. Most hospitals are still usingordinary nasogastric tube decompression, it is confined by the length limit, only forthe stomach and part of the proximal bowel decompression, but in the treatment oflow intestinal obstruction with weak effect. Therefore, we need a way to redeem thedefect of treatment in ordinary nasogastric tube, more thorough decompression.CREATE MEDIC, Japan produced transnasal ileus tube decompression at the site ofobstruction, reduce bowel edema, conducive to the resumption of intestinal blood flow,at the same time with a certain diagnostic purpose, and directly angiography on thesite of obstruction. According to Japanese statistics application of ileus cathetertreatment in intestinal obstruction, surgical treatment rate has been reduced to8.9%.Objective Explore by the value of the of transnasal ileus tube in theconservative treatment of small intestinal adhesion ileus. Methods To explore the efficacy of transnasal ileus tube small intestinaladhesion ileus, in order to confirm the diagnosis in Jilin University Third Hospital,from February2010to January2012,60patients were treated with small intestinaladhesion ileus as the object of study. Treatment divided into the experimental group(transnasal ileus tube treatment group)30cases, the control group (nasogastric tubetreatment group)30cases. Reduce the extent of the patients in the first24hoursabdominal circumference, abdominal pain, bloating to defuse the situation and restorenormal time, the amount of the first24-hour decompression, the orthostatic abdominalplain film performance, independent recovery exhaust time, white blood cell recoverystatistics of normal time, the operation rate, the average hospitalization days.Comparative analysis of the therapeutic effect of the two treatment groups, Confirmedby transnasal ileus tube value in the treatment of small intestinal adhesion ileus.Result Experimental group in30patients,25patients (83.3%) were recover,the control group of17cases (56.7%) in the30patients cured, transnasal ileus tubecure rate was significantly higher than ordinary nasogastric tube. Indicators of theexperimental group and30patients into the transnasal ileus tube:24h-48h abdominalpain, bloating remission rate of90%, abdominal pain, bloating relief to be36.71±7.39hours, the independent exhaust, defecation required5.70±1.47days,24habdominal circumference reduced11.91±2.53cm, the first24hours decompressionamount of1405.00±379.28ml. White blood cells back to normal to be4.39±1.22days, X-line liquid-gas plane disappeare required8.62±1.97days, the operation rate of16.7%, the average hospital stay was15.08±2.78days. Statistics and reference groups,all contrast with a significant difference, P <0.05.Conclusion transnasal ileus tube played an important role in the treatment ofsmall intestinal adhesion ileus. The decompression effect have the advantages of moreeffective, more quickly, more significantly shorten the treatment time, improve theclinical symptoms, lower reoperation rate compared to ordinary nasogastric tube line.transnasal ileus tube in the clinical application of effective ordinary nasogastric tube to make up for deficiencies in the treatment of small intestinal adhesion ileus. It isimportant treatment after inefficacy of ordinary nasogastric tube therapy, but beforesurgical treatment, that is worthy of clinical use.
