Object:We wanted to figure out whether the patients with dilated cardiomyopathy after cardiac resynchronization therapy undergo a change of PLN in the serum, according to the improvement of left ventricular function.Method:Twelve patients with heart failure diagnosed as dilated cardiomyopathy clinically at Zhongshan Hospital of Fudan University were enrolled from July to December in 2008.We collected their history,drug use and echocardiograph data in detail.The serum samples were collected before they were implanted the devices used for cardiac resynchronization therapy(baseline).3 month later,10 of them completed the following up,and the blood samples were collected.The control group consisted of 5 patients who had no history of heart failure,all from the patients hospitalized at the Cardiology Department of Zhongshan Hospital.All serum samples were collected at room temperature,standing 1 or 2 hours. The tubes were then centrifuged at 2000 r.p.m,at 4℃for 10 minutes.Supernatant serum was the immediately placed into Eppendorf tube and frozen -70℃.Until the night before the formal experiment,all the samples would be removed from -70℃refrigerator,and placed in 4℃refrigerator overnight.First of all,determined the best dilution of PLN,then applied it to all the sample.All the specimens were measured the photoelectron energy absorption value after the the ELISA reaction.Through the establishment of the standard curve in the Curve Expert 1.3 software,we calculate the various specimens of the actual concentration of the PLN.All the data was enterd into SPSS 11.5 software for statistical analysis.We used the two-side test,and P values less than 0.05 was considered as statistically significance.Results:In our study,12 patients with CRT therapy group compared with five patients in the control group.There was no significant difference in their age and gender composition,P values were 0.15 and 1.0 separately.At baseline,the mean concentration of PLN in the CRT group was 534.77±264.87 ng/mL,while the mean concentration of the control group was 645.91± 252.61 ng/mL,with no statistically significant difference(P=0.343).At the same time,CRT Group PLN value is slightly lower than the control group.At 3 months follow-up,the patients in the CRT group showed an improvement in the cardiac function,NYHA classification,the serum concentration of PLN decreased significantly(574.80±268.31 ng/mL vs.387.14±223.20 ng/mL P=0.022),but left ventricular end-diastolic diameter did not decrease significantly as compared with the previous(P=0.5).Conclusion:After CRT treatment,the serum content of PLN in the patients with dilated cardiomyopathy,decreased significantly over the previous,which was associated with the improvement of cardiac function,but PLN concentration was no significant difference between heart failure patients and non-heart failure patients. Therefore,there still a lot to be done to figure out the role of PLN in dilated cardiomyopathy.
