The Study On Relationship Of Sera HBVDNA And HBV Serological Makers Associated With Hepatic Tissue CccDNA In HBV Carriers

ObjectiveTo study the relationship of sera HBVDNA and HBV serological makers associated with hepatic tissue cccDNA in HBV carriersMethodsWe screened 30 HBV carriers(including 17 men and 13 women) aged 17-36 years old(average 22.5) who were admitted to Sheng Jing Hospital between May 2007 and February 2008.All patients were diagnosed according to Proposal of Prevention and Treatment for Viral Hepatitis amended in the Tenth National Academic Conference for Viral Hepatitis in 2005,hepatitis A,C,D,E and hepatic lesion caused by other factors excluded.All patients do not receive antiviral treatment six months before hospitalization.Quantitative PCR were used to detect HBVcccDNA in liver biopsies. The same method was used to examine HBVDNA in sera and liver biopsies. Quantification of the HBsAg 'HBeAg in sera were quantified using Chemiluminescence immunoassay.liver tissue inflammatory activity grading(G) and degree of fibrosis staging(S) were evaluated.Results1.The results of hepatic tissue pathology in 30 chronic HBV carriers is:GO/SO,9 (30%),GO-1/SO-1,19(63.3%),G1-2/S1-2,1(3.3%),G1/S3,1(3.3%).2.Quantification of the cccDNA and HBVtDNA level in the tissue:Quantification of the cccDNA:3.15×10~3copies/ml-1.06×10~7copies/ml(logarithm value:5.66±0.93); Quantification of the HBVtDNA:3.99×10~3copies/ml-6.26×10~8copies/ml(logarithm value:7.45±0.18);There was a positive correlation between the cccDNA and HBVtDNA(r=0.375,p＜0.05).3.Quantification of the HBVDNA,HBsAg and HBeAg in the serum: Quantification of the HBVDNA:1.12×10~3copies/ml-6×10~8copies/ml(logarithm value:6.33±1.52),Quantification of the HBVDNA is over10~6 copies/ml is defined as high-load,19(65.5%).The average value of sera HBsAg quantification is 762±201.35 IU/ml;The average value of sera HBeAg quantification is 1065.80±679.84 IU/ml. There was no correlation between the cccDNA level in the tissue and sera HBVDNA quantification(r=0.174,P＞0.05).There was no correlation between the cccDNA level in the tissue and sera HBeAg qantification(r=0.152,P＞0.05).There was a positive correlation between quantification of hepatic tissue cccDNA and sera HBsAg quantification(r=0.862,P＜0.001).Conclusions1.Although the level of the sera HBVDNA is high,inflammation and fibrosis in liver tissue are most belowe 2 degree in HBV carriers whose age belowe 40 and ALT is normal.2.the correlation between the cccDNA level and HBVtDNA level in the tissue is not strong,so we cannot directly infer hepatic tissue cccDNA level from HBVtDNA level in the tissue.3.There was no correlation between quantification of hepatic tissue cccDNA and sera HBVDNA quantification,we cannot casually infer hepatic tissue HBVDNA level from sera HBVDNA level But the level of sera HBsAg quantification may reflect quantification of hepatic tissue cccDNA to some extent.
