BackgroundArrhythmia referrs to the abnormality of the frequency, rhythm, and origination of the heart impulse, and is one of the frequently occurred cardiovascular diseases. The premature beat is the most common type of arrhythmia, which could be classified according to variation of origination as atrial, ventricular and atrioventricular junctional, among which ventricular premature beat appears to be the most common type, then atrial, and atrioventricular junctional premature beat is relatively rare. Healthy people may occur premature beat, usually with coronary heart disease, rheumatic heart disease, hypertensive cardiopathy and myocardiosis, etc. Also, the premature beat could be seen in poisoning by quinidine, pronestyl, digitalis, or low potassium concentration, cardiac operation that gives heart mechanic stimulation, etc. Current consensus agrees that increasingly sensitive automaticity, reentry impulse and triggered electrical activity serve as the mechanisms of the premature beat. According to the origination, the premature beat could be classified as sinus premature beat, sinus-atrial premature beat, atrial premature beat, atrioventricular premature beat, ventricular premature beat. From the incidence point of view, premature beat takes 12. 3% to 27. 2% of the arrhythmia, 3. 6% to 5. 0% of the ordinary population occurs premature beat. And the ventricular takes the most part of the occurence, thenatrial, and for sinus seldom observed. Although new drugs have been consecutively invented, their application is somewhat clinically limited due to the mechanical complexity, potential of leading- to or aggrevating arrhythmia, individually varaity of use. Chinese medicine shows its priority of safety and low-in-side-effect, however some problems are inevitable. My advisor Cheng Wei combines his years of experiences during when absorbed merits of medical practitioners from ancient times to nowadays, from domestic to international, and current researching achievements. He is experienced in theoratically perspecting premature beat both in Chinese medicine and western medicine.ObjectivesThis paper makes retrospective summarization the current strateges of diagnosing and treating premature beat and offers a systematical introduction for Prof. Cheng's over 20 years' experiences in perspecting and treating premature beat, as providing guidence for future clinical practice.Methods(1) Review literatures, retrospect the diagnosis and treatment for premature beat overally;(2) Practise with my advisor, make theoratical combination with clinical practice, as well as records;(3) Test the skills learned through clinical means, compile systematically the records and summarize the academic thoughts of Prof. Cheng.ResultsSystematically retrospective review of the literatures have been summarized for perspecting concept and treatment of the premature beat. Meantime, through studying with Prof. Cheng clinically and reviewing the records, the unique ways of diagnosing and treating premature beat of my advisor have been revealed.ConclusionsThe diagnosis and treatment of the premature beat should be applied detailed Bianzheng Lunzhi, constration of certain hurb or prescription should be avoided and should be applied flexibly. Prof. Cheng emphasizes pulse examination in diagnosis, wind evil in etiology, combination of Bianzheng and disease differentiation in treatment, as reveals the rich scientific connotation inherited in the ancient Chinese medical theory, as well as embodying the "Treatment according to the essence of the disease" philosophic point of view.
