Clinical Research on Unstable Distal Radius Fractures by diaplastic External Fixator1 BackgroundThe distal radius fracture is a common injury in clinic,especially comminuted fracture of distal radial metaphysis is unstable.There is evidence that osteoporosis is present in older patients,especially women,who sustain a unstable distal radial fracture.Open reduction and plate osteosynthesis as well as the application of external fixation are considered to be the most appropriate fixation techniques in the treatment of these fractures in the elderly.There are some shortcomings such as unreliable fixation,re-displacement,many complications and unsatisfied functional recovery in traditional conservative treatment, and some problems such as surgical injury,much expense,many complications and unsatisfied effects in internal fixation operation. Therefore,how to make unstable distal radial fractures standardized and have characteristics and efficacy of Chinese medicine treatment advantages,are now before us an important question.2 PurposesBy treating the unstable fracture of the distal part of radial with diaplastic external fixator,the aim of the research is to set up the treating system(including the safety of external fixator and selection of therapeutic method),and then to establish a systematic,safety therapeutic method in the end,which has the characteristic of Traditional Chinese Medicine and accords with the demands of modern biological fixation.3 Materials and methodsAccording to the paired design,60 patients with unstable distal radius fracture were equally divided into two groups(testing group n=30,control group n=30)from August 2005 to May 2008,which are respectively treated by plate fixation and manipulation-pin-fixator. Including 8 males and 52 females,with an average age of 60.27 years(ranging in age from 34 to 85 years).According to AO classification,Type A3 fracture occurred in 22 patients,Type B2 fracture occurred in 4 patients,Type B3 fracture occurred in 2 patients,Type C1 fracture occurred in 19 patients,Type C2 fracture occurred in 8 patients,Type C3 fracture occurred in 5 patients.the data which using for the anatomic and clinical criteria was collected at 3th month,6th month,15th month.The records was analyzed under statistics analysis.4 Result4.1 According to The Betra's Clinical assessment.testing group in radiological assessment gains 86.27 scores,effective rate is 86.67%;in functional assessment gains 94.93 scores,effective rate is 100%. control group in radiological assessment gains 91.27 scores,effective rate is93.33%;in functional assessment gains 94.23 scores,effective rate is 100%.4.2 The radiological assessment with surgical treatment in the testing group and control group equally gains a better improvement(p<0.01).4.3 In radiological assessment,there is no significant difference between testing group and control group.4.4 The functional assessment with surgical treatment in the testing group and control group equally gains a better improvement(p<0.01).4.5 In functional assessment,there is no significant difference between testing group and control group.5 Conclusions5.1 Compared with the open plate fixation,the unstable distal radius fracture treating by diaplastic external fixator can be achieved a similar result.Its effect should be affirmed.5.2 Using diaplastic external fixator in treating unstable distal radius fracture can use the theory of tendon reinforced bone to expain it.5.3 Continuous traction by diaplastic external fixator is conducive to the restoration of radial length.5.4 Based on advantages with External fixation treatment system,which is easily operated,minimally invasive,less money be used and the second surgery be avoided,and so on.so it is easy to promote the use.
