ObjectiveTo investigate the kinds of pathogenic bacteria detected from the blood culture of patients in hematology and ontology ward, and to observe the drug resistance during the past 7 years.Materials and MethodsThe blood samples of 3,264 pediatric patients with severe infection from 2002 to 2008 in hematology and ontology ward of Zhejiang Children's Hospital were cultured in Bact/ALTER 3D blood culture system,VITEK 60 automicroscan were used to identify the species and to conduct drug resistance test. The results were interpreted according to the guideline of National Committee for Clinical Laboratory Standards.ResultsDuring the past 7 years,Gram- negative bacteria remained the more frequent pathogens than Gram-positive bacteria in the ward. The detection rate of Gram-positive bacteria increases by years. There are more detection in Coagulase Negative Staphylococcus(CNS) than in Staphylococcus aureus. The drug resistance rate of staphylococci against penicillin and erythromycin were more than 90%and 80% respectively. The oxacillin resistance rate of CNS was 89.74%,significantly higher than that of Staphylococcus aureus(9.09%).2 cases with CNS involvement were found to be resistant to vancomycin, but no Staphylococcus aureus and Enterococcus were found to be resistant to this agent. The Extended-Spectrumβ-Lactamases(ESBLs) production rate of Escherichia coli and Klebsiella pneumoniae was 52.29%.The drug resistance of the bacteria with ESBL production was higher than those without. The resistance rate of Escherichia coli,Klebsiella pneumoniae and Pseudomonas aeruginosa to ampicillin and first generation cephalosporins was more than 90%,whereas they were sensitive to imipenem, only 2 cases of Pseudomonas aeruginosa was found to be resistant to imipenem.Conclusion(1) Gram- negative bacteria are the predominant pathogen in our ward which made up by Escherichia coli,Klebsiella pneumoniae,Psudomanas aerugirtosa and some other kinds of Gram- negative bacteria, and the majority of Gram-positive bacterium is Staphylococcus.(2)Monitoring the trends of antimicrobial resistance is very important in guiding clinical therapy.
