Early Clinical Application Study Of Tetraflex Accommodative Intraocular Lens

Purpose To investigate the distance and near acuity and the amplitude of accommodatation after implanting the Tetraflex accommodative intraocular lens (IOL),comparing with those patients who inplanted the AR40e conventional monofocal IOL,and to evaluate the security, accommodated capability and Subjective visual function.Material and Methods All of these 15 patients(20 eyes) in this prospective study were senile cataract and met the inclusion and exclusion criteria during August 2008 to January 2009. Patient chose the type of Intraocular lens by themselves. All these 20 eyes were Peorfmred with Phacoemulsification and implanted one of two types of the IOL. 7 cases of 10 eyes implanted Tetraflex accommodative IOL (study group)and 9 cases of 10 eyes implanted AR40e monofocal IOL(control group). Patients were examined the uncorrected and corrected distant visual acuity, uncorrected and corrected near visual acuity,and the distance corrected near visual acuity at 1 week,1 month,3 months after the surgery,and the amplitude of accommodation by 3 different methods (the dynamic retinoscopy method,defocus method and near point method) at 1 month,3 months after the surgery. Besides, patients were also examined the change of the anterior chamber depth(ACD) after pharmacologic stimulation of 2% pilocarpine eyedrops and made questionnaire to assess subjective visual function at 1 month and 3 months after the surgery. The results of the study group were compared to those of the control group and statistical analysis was performed using SPSS Advanced Statistical 13.0 Software. Results At 1 week,1 month,3 months after the surgery,there were no statistical significant between the study group and the control group about the uncorrected and corrected distant and near visual acuity(F=2.436,p=0.136;F=0.762,p=0.394;F=3.338, p=0.084; F=1.740,p=0.203).But the distance corrected near visual acuities were significantly better in the study group than in the control group(F=11.524,p=0.003). At 1 month after the surgery, the amplitude of accommodation by 3 different methods (the dynamic retinoscopy method,defocus method and near point method) were (the study group versus the controp group) 0.89±0.22D versus 0.73±0.17D,1.3±0.35D versus 0.83±0.27D, 1.63±0.70D versus 0.95±0.18D; at 3 months after the surgery were 0.98±0.25 D versus 0.52±0.18D,1.125±0.21D versus 0.6±0.21D,1.33±0.32D versus 0.72±0.21D; the study group significantly higher than the control group(F=39.304, p<0.001;F= 28.685,p<0.001; F=16.756, p=0.001). The change of the anterior chamber depth(ACD) after pharmacologic stimulation of 2% pilocarpine eyedrops were no statistical significant between the study group and the control group(1 months after the surgery: 0.44±0.28mm versus 0.26±0.23mm;3 months after the surgery: 0.40±0.24 mm versus 0.27±0.25mm; F=2.902,p=0.107). In the subjective evaluation of visual function, the study group got higher score than the control group in the visual function questionnaire survey (1 months after the surgery: 86.59±7.51 versus 77.69±8.89;3 months after the surgery: 86.14±8.20 versus 76.15±11.4;F=5.962,p=0.025),and the number of patients who don't need glasses during daily life was 6 patients in the study group and 2 patients in the control group (p=0.085).Conclusion There were no statistical significant between the Tetraflex accommodative IOL and the AR40e monofocal IOL about the uncorrected and corrected distant and near visual acuity,but the Tetraflex accommodative IOL significantly improved the distance corrected near visual acuities,increased patients'accommative amplitude and ameliorated the subjective visual function. Its long-term effect remains to be observated.
