Objective: to analyze inflammatory reaction in the early stage of lumbar herniated disc tissue under cyclooxygenase-2 (COX-2) inhibitor therapy, and the influence of COX-2 inhibitor in reabsorption of herniated discs tissue.Methods: Thirty two New Zealand rabbits were used and divided randomly into drug group and non-drug group(control group), sixteen rabbits in each group. A new L5/L6 disc herniation model was made by Yoshida's method, the drug group were given COX-2 inhibitor (Celecoxib) 5mg/kg/day together with feeds.All rabbits were sacrificed after 4 weeks, for half of the L5/L6 discs in each group were harvested, then size of the herniated discs were measured using a special way,and PGE2 content were detected by radioimmunoassay. For the left half, whole L5-L6 were harvested and fixed with l0% formalin for histologic and immunohistologic examination with hematoxylin-eosin stain and anti-rabbit macrophage antibody CD-68,the grade of macrophage infiltration was expressed by negative(-),moderate(+),abundant(++).Results: After four weeks, HE stain showed fibrous granulation tissue invading into disc tissue in both groups. The content of PGE2 in drug group was 36.01±6.90 pg/mg, and 61.07±8.75 pg/mg in control group (P<0.05); The size of the herniated disc in drug group and control group were 2.20±0.18mm2 and 1.94±0.11mm2 (P<0.05) respectively. Macrophages were detected in all of the eight control group herniated disc tissue,in 4 of 8 discs (50%) were found abundant cells(++);in the drug group,2 discs (25%)were(++) and no macrophages were observed in one discs. Conclusion: Celecoxib obviously relieved inflammation reaction induced by herniated nucleus pulposus, and reduced the infiltration of macrophages,which may hinder the spontaneous resorption progress of herniated discs. However, the influence of COX-2 inhibitors on lumbar disc herniation during the early stage needs further study.
