Objective: To explore the efficacy and common influencing factors of hyperbaric oxygen (HBO) therapy on patients with severe traumatic brain injury (STBI).Method: Clinical data of 150 patients with STBI were analyzed retrospectively. HBO therapy was performed in 82 cases and routine therapy in 68 cases at the same time. The efficacy of the two groups was compared. The influencing factors were investigated statistically, including sex, age, cause of injury, Glasgow coma score (GCS) at admission, operation of craniocerebral surgery, incision of trachea, time of beginning HBO treatment after injury and times of HBO treatment, and compared analysis was done according to the main influencing factors. Glasgow outcome score (GOS) at the six-month follow-up after injury evaluated as the criteria was analyzed statistically. Count data were compared withχ2 test and the potential influencing factors were evaluated with the logistic regression.Results: Significant differences were shown between the two groups (P<0.01). The rate of good recovery (GR) of HBO group was higher than that of control group (P<0.05), and the rate of vegetation state (VS) and death (D) was significantly lower than control group (P<0.01). GCS at admission , circumstance of incision of trachea and time of beginning to treat with HBO after injury were the major influencing factors. Higher rate of GR and lower rate of VS and D happened in patients with GCS score more than 5 (P<0.05), no tracheotomy (P<0.01) and HBO treatment less than one month after trauma (P<0.05), respectively.Conclusions: The treatment of HBO is beneficial to patients with STBI. The higher the scores of GCS at admission are and the earlier HBO therapy begins, the better the effect shows. Patients with STBI should receive HBO treatment as early as possible without contraindications.
