OBJECTIVE: To investigate the characteristics of coronary lesions in patients with sudden coronary death and evaluate the role of coronary angiography on risk stratification for sudden coronary death .METHODS: Coronary angiography was performed on 47 consecutive cases(39 males, 8 females, mean age, 62.57±12.67 years) of sudden coronary death or Survivors in the first affiliated hospital of Guangxi Medical University, From January 2000 to March 2009 .RESULTS: 1. One- , two- and three- vessel lesiones were present in 31.9%(15), 25.5%(12), 42.6%(20) patients respectively. 12.8%(6) had concomitant left main stem (LM) disease. 2. Total occlusion (100% stenosis), subtotal occlusion (95% to 99% stenosis), severe stenosis (70% to 94% stenosis) and borderline stenosis (50% to 69% stenosis) of infarct-related artery(IRA) were seen in 57.4%(27), 25.5%(12), 12.8%(6) and 4.3%(2) of the cases respectively. 3. The IRA showed diffuse, tubular and limitations of coronary artery disease , respectively55.4% (26), 19.1% (9) and 25.5% (12). 4. LAD, RCA, CLX of the distribution site of IRA were present in 31.9% (15), 57.4% (27), 10.6% (5) respectively. Single factorχ2 test shows that location ,the severity of stenosis and the length of lesion of IRA enter the formula (P<0.05), the number of vessel lesions also enter the formula (P<0.05) ;In multi-factor logistic regression analysis, the severity of stenosis of IRA and the number of vessel diseases are associated with a increased risk of SCD.CONCLUSIONS: (1) The study shows that patients with sudden coronary death usually suffer multi-vessel lesions with severe lesions of IRA; (2) Coronary angiography may serve as a indicator on risk stratification for sudden coronary death.
