Objective:To study the hypoglycemic effect of gastric bypass(GBP) on streptozotocin(STZ) -induced diabetic rats and its mechanisms.Methods:1.After adaptive feeding week,They will be randomized according to the weight and their model is divided into 25 and non-building modules 25.non-building modules Feed basic animal feeds,normal diet for 8 weeks.Model group is given high sugar high fat diet,and after4 weeks is given intraperitoneal injection 30mg/Kg by 1%of the STZ solution(PH4.4 0.1mmol / L of sodium citrate - citric acid buffer fresh ice bath configuration).after a week, we pick two consecutive fasting blood glucose value of≥7.8mmol / L in rats as a model to continue the high-sugar high-fat feeding to four weeks,then measure in rat tail vein blood glucose> 13.6mmol / L and the stability of 5 days as the successful model.a total of 23 becomes into mold2.Grouping of non-experimental animals were randomly divided into control group(one group) and surgical group(group 2),the model were randomly divided into model(3) and model + surgery group(Group 4).10 in each group(the remaining rats standby).Preoperative 1 group and 2,3 group and group 4 weight,fasting blood glucose,the average food intake was no significant difference.3.intervention groups,group 2 and group 4 operate GBP surgery,preoperative ban eating rats 12h,open water,0.5%sodium pentobarbital(50mg/kg) intraperitoneal injection on the operating table after anesthesia fixed,aseptic conditions,on the middle abdominal incision into the abdominal incision 3cm long,closed distal stomach,cut off 10cm from the Treitz ligament jejunum,distal jejunum and proximal anastomosis the posterior wall of the stomach and from gastrointestinal anastomotic line jejunum 10cm distal end-to-side anastomosis,after the completion of 2ml were washed with gentamicin,abdominal closure.4.Detection of preoperative and postoperative 48th hour,1 week,8 weeks measure fasting blood glucose,insulin,glucagon-like peptide -1(GLP-1) rats are fasted overnight,tail vein blood 1.5ml,3000r/min centrifugal 15min after standing 30min.the separation of serum detects fasting blood glucose,insulin,glucagon-like peptide-1(GLP-1).Results:1.GlucoseCompared with the preoperative,group 4 after GBP the first week there is statistically significant decline in blood glucose(P <0.01),after eight weeks blood glucose was significantly lower than Group 4 with the point in time model(group 3)(P <0.01 ),with the preoperative group was also markedly decreased compared(P <0.01),operation group (group 2) preoperative and postoperative changes in blood glucose was not significant(P> 0.05)2.InsulinCompared with the preoperative,after one week the 4 group GBP fasting insulin values from(28.95±3.99) mIU / L increased to(33.83±5.61) mIU / L(P <0.05), after 8 weeks further increased to(60.05±8.60) mIU / L(P <0.05).3.glucagon-like peptide -1(GLP-1)Compared with the preoperative,after one week the 4 group GBP fasting glucagon-like peptide -1 by(7.30±1.88) pmol / L increased to(13.02±4.30) pmol / L(P <0.05) after 8 weeks further increased to(20.60±7.61) pmol / L(P <0.01)Conclusion:In this study,type 2 diabetes animal model,confirmed GBP on type 2 diabetic rats with good blood glucose control,group 4 one week after surgery by fasting blood glucose value (17.5±0.8) mmol / L decreased to(11.0±0.6) mmol / L(P <0.01).To the end of the experiment blood glucose without a rebound,indicatesthat GBP surgery on type 2 diabetic rats with good hypoglycemic effect.According to Rubino,such as research GBP of diabetic rats with blood glucose control to reduce the weight loss after diet has nothing to do.And after this experiment GBP rat GLP-1 from(7.30±1.88) pmol / L increased to(13.02±4.30) pmol / L(P <0.05) after 8 weeks further increased to(20.60±7.61) pmol / L(P <0.01).prompted after GLP-1 increased GBP may improve blood glucose in diabetic rats may play an important role.Group 2 before and after surgery no significant changes in blood glucose values,2 Group 1 Group with blood glucose levels and no statistically significant changes in the operation for the normal rats had no effect on blood sugar,indicating that the operation would not affect the normal blood glucose levels.It is of important significance for the treatment of diabetic GBP mechanism further study.
