Meniscus injury is one of the more common knee injury sites.Because the type of meniscal injury is diverse and often cause symptoms in patients with varied,it difficult to make accurate diagnosis for clinicians.Therefore,this study collected 153 cases of patients with meniscus injury information through October 2003 to October 2008,and further retrospectively analyzed the morphology and classification of meniscal injury.To compare the initial clinical,arthroscopic and magnetic resonance imaging(MRI) on the diagnosis of meniscus injury,which is benefit to clinical diagnosis of meniscus injury.Objective To explore the morphology and classification of meniscal injury and the accuracy of clinical,arthroscopic and MRI in the diagnosis of meniscus injuries and their instructional value for clinical management.Methods Clinical,arthroscopic and MRI imaging of 153 meniscus were retrospectively analyzed.We explore the morphological image and classification of meniscus injury through arthroscopy,Using MRI as the diagnostic standard,the sensitivity,specificity and accuracy of clinical and arthroscopic were valued.Results In accordance with morphological features of meniscus injury,it can be divided into six categories:longitudinal tear,horizontal tear,radial or oblique tear, discoid meniscus,meniscal cyst,and composite types of injury.The sensitivity, specificity and accuracy of clinical in diagnosing meniscus injuries respectively were 88.9%,50%,87.9%,comparing with MRI,there was a significant difference(χ~2 = 10.3, P<0.01).And that of arthroscopy were 66.7%,75%,67.5%,comparing with MRI,there was a significant difference(χ~2=48.1,P<0.01).For meniscus rupture,there was no difference between arthroscopy and MRI(χ~2 = 3.3,P>0.05).Conclusion In order to increase the accuracy of diagnosis of the meniscus injury,clinical diagnosis should be combined with MRI,MRI is of important clinical value in the diagnosis of the meniscus injury of the knee joint,arthroscopy should not be used as a routine method in diagnosis of the meniscus injury,And its diagnosis and surgical indications should be in the light of clinical diagnosis and MRI showing levelⅢ.
