Clinical Research Of Acute Ischemic Apoplexy Of Brain And Microalbuminuria' Level

Objective:1,To explore the relationship between microalbuminuria and ischemic acute stroke,risk factors of stroke.2,To investigate the relationship between microalbuminuria and cerebral macroangiopath,cerebral microangiopath in Ischemic Stroke patient.3,Exmine the relationship between microalbuminuria and condition in Ischemic Stroke patient.methods:We studied 98 patients(from 50 to 75 years) diagnosed with ischemic acute stroke confirmed by computed tomography or nuclear magnetic resonance.The control group consisted 90 age-and-gender-matched subjects(from 50 to 75 years),diagnosed with non-acute-stroke.We excluded patients with:liver and renal inadequacy,urinary system infection,malignant tumor,vasculitides or renal insufficiency,urine protein masculine,cerebral arterial thrombosis is diagnosed according to all kinds of cerebrovascular disease diagnosis main points,which was passed by Fourth National Institute of cerebrovascular disease.In all patients the completion of blood routine,blood pressure,blood lipids,blood glucose,liver and kidney Gong,electrolytes,ECG and C-reactive protein to check.Cerebral arterial thrombosis is confirmed by Head CT or MRI.Student's t tests were used to test if there were significant differences in continuous variables between patients with and without microalbuminuia.X~2 test were used to test if there were significant differences in categorical variables between patients with and without microalbuminuria.logistic regression analysis was used to identify significant independent predictors of the time to development of either new stroke or new microalbuminuia.If essenttial analysis of variance and Linear regression will be used.The severity of the neurological deficit was assessed by the Scandinavian Stroke Scale(SSS).The albumin excretion rate was measured in daily urine collection,using the immunonephelometric method.Results:1,MA was found in 44 of(45%) acute stroke patients and in 22 of 90(24%) controls,There were significant differences in statistics(p＜0.05).Microalbuminuia,incidence rate cerebral macroangiopath in Ischemic Stroke is 60%,compare with cerebral microangiopath,29%,there were significant differences in statistics(p＜0.05).2,Patients with MA in acute stroke often is associated with diabetes,hypertension,coronary heart disease,C-reactive protein,low density lipoprotein cholesterol.At the same time,the final step results of logistic regression analysis shows that diabetes,hypertension,coronary heart disease,C-reactive protein,low density lipoprotein cholesterol are final variable entering equation.They are risk factors of strok,there is a positive correlation between microalbuminuia and the risk factors,we find that diabetes,hypertension,coronary heart disease,C-reactive protein,low density lipoprotein cholesterol are risk factors of MA.3,In cerebral macroangiopath of ischemic stroke patients with MA scored lower on the SSS than patients without MA,on admission,14th day,30th day,the differences were statistically significan(P＜0.05 ),in cerebral microangiopath of ischemic stroke patients with MA scored lower on the SSS than patients without MA on admission,the differences were statistically significant(P＜ 0.05).In cerebral macroangiopath of ischemic stroke patients with MA scored lower on the barthel index,on admission,21th day,the differences were statistically significant(P＜0.05 ).4,In cerebral macroangiopath of ischemic stroke patients with high MA scored lower on the SSS than patients without low MA.5,Study group have high CRP,compared with control group.Conclusion:1,We found that MA can be detected in about 45%of patients with acute ischemic stroke,there is significant difference compared with control group,it manifests patients with acute cerebral arterial thrombosis often complicate with microalbuminuria,patients in cerebral macroangiopath of ischemic stroke have high incidence rate about microalbuminuria.2,Microalbuminuria is correlative with diabetes,hypertension,coronary heart disease,C-reactive protein,low density lipoprotein cholesterol.3,In cerebral macroangiopath of Ischemic Stroke patients with MA have scored lower on the SSS or barthel index than patients without MA.In cerebral macroangiopath of ischemic stroke patients with high MA scored lower on the SSS than patients without low MA.
