Clinical And Experimental Study Of Ameloblastoma Treated By Decompression

Objective:To value the clinical effect of ameloblastoma treated by decompression, and to explore the BCL-2 and P53 protein expression changes before and after the decompression in ameloblastoma.Explored the repair mechanism of the decompression that provide the theory basis for further clinical effect,molecular biology and pathology investigation.Methods:40 cases was chosen from May,2007 to December,2008 in the Affiliated Stomatology Hospital of Kunming Medical University whose X-ray show a Mandibular cystic lesions.The 40 cases treated by decompression under local anesthesia.15 cases were diagnosed as ameloblastoma by Pathology and selected in our study.Compared the cystic size by X-ray before and after surgery in 3 months,6 months and 12 months to evaluate the clinical effect.The 15 cases in experimental group were cut small specimens in the third month after surgery.The expression of Bcl-2 and p53 protein were detected by immune histochemistry(S-P) in all cases.Results:The 15 cases followed-up period of 3-12 months,11 cases show a remarkable lessen by X-ray,the effective rate was 73.3%,2 cases have no change and 2 cases deteriorated.The positive expression of Bcl-2 protein in the 15 cases was 86.6%(13/15),with performance of yellowish brown pellet in cytoplasma,distribution of focus or diffuse;The positive expression of p53 protein is 60%(9/15),mainly located in the cell nucleus.After three months by decompression,the expression of Bcl-2 and p53 protein were down regulation.(bcl-2:86.6%to73.3%;p53:60%to 53.3%).Each two aspects of these two protein had statistics significance(P＜0.05). Conclusions:(1)Decompression is one of the simplest and effective treatments forameloblastoma.(2)As a device to maintain the wound,hollow removable appliance has some advantages,such as easy washing and not easy infection.(3)Bcl-2,P53 protein play an important role in the occurrence and development in ameloblastoma.(4)After the decompression,the protein expression of Bcl-2 and p53 were down regulation which induced the tumor cells apoptosis.
