| Objective and Backgroud:The primary hepatocellular carcinoma is a common digestive system malignant tumor, the morbidity of which is high, and the treatment effect is poor. It is one of the common malignant tumor that affect people's health seriously. So far, the first-selected therapeutic method of early hepatocarcinoma is surgery. The interventional therapy is the first selection for moderate and andvanced stages of hepatocarcinoma. The interventional therapy have made obvious progress in inhibiting tumour from growing, improving patient's survival rate etc, but the results of decline of patient's organism immunity and liver function, tumor cell insensitive to chemotherapy medicine able to bear medicine and postoperative tumor bypass circuit circulation set up have made the effectiveness not good enough.In recent years, because of the constant development of thermotherapy technology, the anticancer result is improving constantly, thermotherapy has alternatives to wound function to the cancer cell, can strengthen sensitiveness to chemotherapy medicine of the cancer cell, it being with get involved thromboembolism treat, combine, can improve therapeutic effect of hepatocarcinoma. The clinical and experimental researches have already proved that thermotherapy combined with interventional therapy not only has a cytotoxic effect of hyperthermia but also increase the sensitivity of chemotherapy, which could increase clinical easing rate, prolong survival period of patients, decrease metastasis and recurrence, and has already achieved approval effects.Method:A total of 60 patients with primary moderate and andvanced stages of hepatocarcinoma were selected in tumor hospital of Jilin Province and divided into two groups ,each group had 30 cases.The observed group was performed interventional hyperthermia chemoembolization with HGC-3000 maked in Zhuhai.The control group involved in the interventional chemoembolization skill for the routine. The treat group was compared with the control group.From observing the chinical effect with the former method and evaluating its advantage and disadvantage, we can fight for applying it in clinic better and make it become the important method of comprehensive therapy to moderate and advanced stages of hepatocellular carcinoma . Use Seldinger's method contrast and hyperselect surpass insert the pipe to artery that surpply blood to tumor, depend on the result of contrast. The dosage was determined according to thegeneral situation and surface area of body of the patient, oxaliplatin 100-120 mg +5% glucose 540 mL, add high pressure of thermotherapy machine inject tube heat, Epirubicin 50 mg +5% glucose 100 mL and Hydroxycamptothecine 20 mg +0 . 9% NaCl 100 mL pour into by conduit, pingyangmycin 8-16 mg + fluidity lipiodol 5-30 mL carry on the blood vessel thromboembolism of tumour , then use gelatin sponge to embolism arteriole . Usually thermotherapy temperature is seted at 48℃~60℃, the velocity of flow is 0. 3 mL/s, and pouring time is 30 min .The treatment group which has 30 patients has experienced 1 to 4 therapia course .Control group experienced common interventional therapy .After treatment, compare two groups' objective effect, life quality improving situation, liver renal function, blood routine, AFP, T cell's subgroup ,NK cell and side reaction.Results:1. treat group and control group lipiodol depositing effective power is:93.3%, 70% respectively . the life quality improved efficient of the two groups is: 80%, 56.7% respectively .2. AFP change situation of the two groups: The effective descent rate of Two groups are 86.7%, 60% respectively.3.tumour blood capillary change situation After thermotherapy :The middle-temperature thermotherapy group can increase the tumour blood capillary compared with high,low-temperature .the high-temperature thermotherapy group can reduce the tumour blood capillary .4.T cell's subgroup ,NK cell change situation of the two groups:treatment group'CD3~+,CD4~+,CD4~+/CD8~+ and NK cell are more than control group .5.The survival rate of 1,1.5,2 years are higher than control group .There is no obvious difference in the side reaction.Conclusion: The interventional hyperthermia chemoembolization is a effective method for the moderate and advanced stages of hepatocellular carcinomacan which not only shows the effects of the byperthermia in killing neoplastic cells, improves the curative affects of the interventional heated chemotherapy only, but also reduce the blood vessel regeneration of tumor, inhibits tumor from growing and shifting, enhances their immutability ,increasing the efficiency of therapy , improves the life quality of the cancer sufferers ,prolongs the survival period of the patients.
