Objective Through studying the clinical data of the diagnosis and treatment of renal cell carcinoma and follow-up study of these patients in Shandong Province hospital of Shandong University during the past 15 years,to find out the component trends of the disease,summarize the evolvement of diagnosis and treatment of the disease,discuss the method how to prophylaxis,detect as soon as possible and treat the disease and to improve survival rate.data and Methods There are 16216 cases reviewed with standard survey tables, which were hospitalized to urological department of Shandong Province Hospital of Shandong University from Jan.1993 to Jan.2008.Inclusion criteria:the patients suffering renal cell carcinoma which were treated in Shandong Province hospital of Shandong University during the past 15 years.There are 483 cases selected in total 16216 cases according to the above standards.The 15 years were separated into 3 periods:mid of 90's age(1993—1998);end of 90's age(1998—2003);new centurial years(2003—2008).Through calculating the average,proportion,median and ratio of these collected cases,we could analysis and statistize renal cell carcinoma's tendency and variation etc.to get some useful information to guide our clinic.Results in the three ages,the number of average patients per annum respectively is 475.2,796.8 and 1971.2;the average hospitalization rate per annum respectively is 2.35%,2.76%,3.25%.In different age the cases number and ratio of identical age patients show a raised trend.In mid of 90's age,the episode age centralizes between 41 and 50;In end of 90's age,the age is centralized between 45 and 55,and the ratio of patients beyond 60 increased continuingly;in new centurial years,the age is centralized between 55 and 65,the ratio of patients beyond 60 increased continuingly.clinical manifestation:177 cases had haematuria(36.6%),with 45 cases of microscopic hematuria;89 cases had lumbago or bellyacke(18.4%);44 cases had abdominal mass(9.11%);.19 cases had " triad of renal cell carcinoma"(3.93%).And other manifestation:59 cases with hypertension(12.2%);44 cases with fever(9.1%); 43 cases with anaemia(8.9%);40 cases with asarcia(8.3%).accessory examination:Diagnosis was made by ultrasonography and CT scanning. Diagnostic rate by ultrasonography was 97.5%,and 8 cases with cancer embolus of venae cava inferior were showed by ultrasonography,with the misdiagnosis of 7casas; Diagnostic rate by CT scanning was in the rate 98.4%with the misdiagnosis of 3 cases;IVUorKUB was also used to diagnosis with the rate in 77.6%;MRI was used to exam the cases with cancer embolus of venae cava inferior.332 cases underwent radical nephrectomy,56 cases underwent unnormalized radical nephrectomy 14 cases underwent partial nephrectomy,3 cases underwent palliative nephrectomy and 15 cases had no-opration.47 cases underwent radiotherapy after operation,176 cases underwent treatment of IF-α,and 92 cases underwent chemotherapy after operation.Pathology findings:there were 392 cases of clear renal cell carcinoma(89.50%),the number in each age-period is increasing;26 cases of papiliform renal cell carcinoma;19 cases of the mixed type renal cell carcinoma.Follow-up was achieved in 322 cases,with the 3,5year and 10 year survival rates being 72.7%(157/216),52.5%(67/139),19.1%(8/42) respectively.There are 37.2%of patients continue to be treated after discharge;There are 65 patients having periodic review.Conclusions With the development of the age,the average hospitalization per annum is gradually increased.The average hospitalization rate per annum of renal cell carcinoma is gradually increased.The episode age of renal cell carcinoma is ageing."triad of renal cell carcinoma" becomes less.Clinical manifestation of renal cell carcinoma is atypical,ultrasonography and CT scanning remain important diagnostic tools currently available in the diagnoses of renal cell carcinoma;while radical renal nephrectomy or partial nephrectomy continues to be the most effective approach to deal with this malignancy.The patients' review and treatment after discharge are very poor,so it is very important to enhance publicizing the knowledge and significance of treatment of renal cell carcinoma.
