The Clinical Observation And Tentative Research For The Tongue Diagnosis And The Pulse Diagnosis Of Chest Stuffiness And Pains

Objective:By observing tongue pictures and pulse conditions of patients with Chest Stuffiness and Pains before and after improvement of symptoms after therapy, in order to investigate the characteristic features, finally to explore the specific rule of characteristic changes of tongue pictures and pulse conditions before and after improvement of symptoms after therapy.Methods: Cases according with Chest Stuffiness and Pains of deficiency of both qi and yin from TCM diagnose and unstable angina pectoris from Diagnostic Criteria, strictly corresponding to inclusion criteria and inclusion criteria were chosen from The Ward of Cardiovascular department of First Affiliated Hospital of Liaoning University of TCM, totally coming to 136 incidents. Main symptoms, secondary symptoms, tongue pictures and pulse conditions of included patients before and after improvement of symptoms after therapy were recorded in detail,then the main symptoms and secondary symptoms were given value assignment and marks, finally the database of the patients symptoms was established. The collection of data of all the included patients are based on the materials at the second morning after hospitalization and the morning of discharge (convenient to collect tongue pictures and pulse conditions). In the observation indexes, tongue pictures included tongue color, tongue body, quality of tongue coating and color of tongue coating; pulse conditions included pulse position, pulse rate, pulse width, pulse length, pulse fluency, pulse tensity and pulse evenness. The database got introduced into statistical software SPSS 15.0, and carried out Wilcoxon Matched-pairs Signed-ranks Test, in order to search the correlation between tongue pictures, pulse conditions and Chest Stuffiness and Pains. Exploring the specific rule of characteristic changes of tongue pictures and pulse conditions should be under the direction of the changes of symptoms of patients.Results: Chest Stuffiness and Pains of deficiency of both qi and yin(unstable angina)mainly red tongue and crimson tongue, at the onset of the disease mainly crimson tongue; after therapy the symptoms gradually improved and the tongue color also gradually became light, mainly red tongue. At the onset of the disease tongue coating mainly yellow thick and dry fur, the proportion of the less fur less than 30%; after therapy with the improvement of the symptoms, thick fur gradually thinned out, the color of tongue coating gradually turned white, dry fur also had improvement in various degrees, tongue coating of patients with less fur also gradually grew out. In addition, in such patients the large fat tongues were primary, a few were little thin tongues, more than 40% tongue body accompanied with indentation, more than 30% accompanied with crack. After therapy, indentation and crack were seen no evidently reduced.During acute attack,Cases with Chest Stuffiness and Pains of deficiency of both qi and yin(unstable angina), pulse conditions mainly included deep pulse, rapid pulse, forceful pulse, wide pulse, slippery pulse, wiry pulse, nevertheless, during acute attack, except normal pulse condition, rapid pulse, wide pulse, slippery pulse, forceful pulse are also common to observed. During the remission stage, a few floating pulse, racing pulse, uneven pulse, soft pulse and atony pulse can also be seen. During the remission stage they all turn normal pulse condition. It is noteworthy that there are about one third left cun pulse of such patients weak, part of which improved after therapy. But the pulse rhythm had been seen no evidently changed.Conclusion: The tongue pictures of Chest Stuffiness and Pains of deficiency of both qi and yin: tongue body fat and big, tongue nature crimson, tongue fur yellow thin and dry, accompanying with indentation and crack. After disease remission, tongue nature turn light, tongue coating turn white, thin and humid, and they presented certain law. The pulse conditions of Chest Stuffiness and Pains of deficiency of both qi and yin: During acute attack period the pulse conditions mainly included deep pulse, rapid pulse, forceful pulse, wide pulse, slippery pulse, wiry pulse, after disease remission, pulse rate decreased, wide pulse gradually turn thin, slippery pulse also decrease, pulse force gradually turn weak finally to normal, but that mainly wiry deep pulse, wiry thready pulse, deep thready pulse, deep slow pulse. The changes of the pulse condition suggested the severity of the disease and prognosis.
