Epidemiology Of Patients With Traumatic Brain Injury In The Rehabilitation Center

Objective:To investigate the epidemiologic characters of traumatic brain injury(TBI) within admission patients in the rehabilitation center.Methods:Undergo a retrospective study by collecting the general and clinical information of admission patients who have suffered from TBI in Sub-center of Rehabilitation Department,Huashan Hospital from 1st January 2004 to 31st December 2007.Analyze the data by SPSS 13.0.Results:411 patients were included in this study.Male patients were 4.9 times as many as female ones.The average age was(40.99±15.47) years old.The patients aged 27~45 constituted 43.1%of the all.The patients' main occupations were company staff, workers and retired persons(the ratio is 27.7%,18.5%and 15.6%respectively).The main cause of TBI was traffic accident.Epidural,subdural or intercerebral hematoma occurred most frequently among the patients,following by contusion and laceration of brain,subarachnoid hemorrhage and skull fracture.Most of the patients had coma, brain operation(s) or endotracheal intubation/tracheotomy during the acute phase after TBI.The average time between the date of onset and the date of admission was (96±152)days,and 73.7%patients was admitted less than three months.The average hospitalization time was(70±4) days.Without 114 patients who were in coma or vegetative state,we found 243 patients having motor dysfunction,150 having cognitive dysfunction,139 having speech dysfunction and the others having psychological,swallowing,or other kinds of dysfunctions.Of them all,nearly 3/4 patients had more than one dysfunction.All the patients received medication therapy, and more than 70%patients received Chinese traditional therapy,hyperbaric oxygen therapy,or physical therapy.The amount of patients who needed to receive speech therapy and occupational therapy was less than the amount of patients who really received them.Pulmonary infection occurred in almost 1/3 patients while urinary infection occurred in 1/4 of them during their hospitalization course.18.0%patients were diagnosed as hydrocephalus and 16.3%as post-traumatic epilepsy.Nearly 70% patients got better while about 30%had no progress when they discharged.Conclusions:Male,persons within working age and staff in cities were the main TBI patients in our rehabilitation center.Traffic accident was the main cause of TBI patients receiving rehabilitation.Most patients were in severe situation during acute phase and had complicated conditions when admitted.Without a large amount of coma or vegetative state patients,most had motor dysfunction.We still need to improve speech and occupational therapy,and pay more attention to the possibilities of infection, hydrocephalus and post-traumatic epilepsy during rehabilitation.
